This nursing home ended up receiving rock bottom ratings in our analysis. Perhaps most alarmingly, it received an F for inspections. More often than not, this means we found some red flags on its government inspections. With nursing homes that received this bad of an inspection rating, we recommend scrutinizing any severe deficiencies on its inspection report. This facility had one category J through K deficiency, which rank among the worst deficiencies. These categories mean that CMS uncovered a deficiency which places patient safety or health in immediate jeopardy. Finally, this facility also received 14 substantiated complaints in recent years. This is yet another bad sign.

We also wanted to point out this facility's poor short-term care grade where it received an F. In determining our short-term care grades, we assess the nursing home's skilled nursing services, such as those performed by registered nurses, speech therapists, physical therapists and other types of skilled professionals. The goal is to devise a scale for sizing up the rehabilitation services of various nursing homes. With a grade in this range for this area, we weren't surprised to discover this nursing home was well below average in two of the key staffing areas we focus on. The nursing home provided substantially fewer physical therapist and registered nurse hours per resident than most other facilities. Lastly, we considered the number of residents that ultimately were able to return home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 33.6 percent of this facility's residents were able to return home.

Another concern was the F this facility was given in the category of long-term care. If you are looking for services other than short-term rehabilitation, you should take a close look at each facility's long-term care scores. In addition to assessing the volume of care provided by nurses aids and other staff, we analyzed the percentage of residents vaccinated against pneumonia. This facility vaccinated 100 percent of its patients, which is higher than the vast majority of nursing homes. To our surprise, this place was able to limit hospitalizations. Although it had 2.11 hospitalizations per one thousand long-term resident days, its risk adjusted score was not bad due to it having more complex patients.

The final area we graded is nursing. Sadly, it received a lowly F for this area, which is an abysmal grade. Nursing ratings are tied to nurse staffing levels. With just 2.7 hours of nursing care per resident each day, this facility's nurse staffing levels were well below the national average. Unfortunately, this nursing home also had terrible marks in some of the quality-based measures to go along with its weak nursing hours. We looked at the percentage of patients suffering pressure ulcers and we were disappointed. This place was at roughly 150 percent of the national average in this statistic. This is likely a bad sign in light of the fact that so many bed sores could be avoided with better nursing care. This metric helped pull down this nursing home's nursing rating quite a bit.
