While we were not high on this facility's overall score, it actually received a very impressive inspection report this year. In fact, it received an excellent inspection report this year. As a result, it earned one of our highest grades in that category with an A-. Arguably the most important factor we look at in calculating our inspection ratings is deficiencies. These deficiencies can be found on a nursing home's recent inspection reports. It is typically in your best interest to avoid nursing homes that had a long list of severe deficiencies. This place was hit with 4 deficiencies on its inspection report, but none were considered severe. This tells you that CMS did not deem any of the deficiencies an imminent threat to resident health or safety. A few minor deficiencies are not necessarily the end of the world.

One of the other reasons we graded this nursing home so highly is that it earned a very impressive nursing rating. Nursing turned out to be its second best category grade. In that area, we gave this nursing home an D. The nursing rating assesses many factors, however, the paramount consideration is the quantity of nurse hours spent with patients. This nursing home averages 3.4 hours of nursing care per resident per day, which is below the national average. To go along with its subpar totals in the area of nursing hours per patient, this facility also didn't perform as well in the quality-based measures we looked at in computing our nursing grades. We looked at the percent of patients experiencing major falls and pressure ulcers. This facility had more pressure ulcers and falls than the average facility.

We gave this nursing home a grade of just D for our short-term care grade. This is not one of our better scores. In the category of short-term care, we seek to assess indicators of a facility's rehabilitation services. We analyze a facility's skilled nursing services, such as those performed by registered nurses and physical therapists. With a grade this poor for this category, we were not shocked to learn this nursing home was below average in multiple staffing areas we focus on. It offered far fewer registered nurse and physical therapist hours per patient than most other facilities. Finally, we considered the percentage of residents who returned home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 40 percent of this facility's residents were able to return home.

The last category we assessed is long-term care. This facility was given a poor grade of F in this area. In a long-term care setting, the primary objective is to maintain patients' quality of life and keep them safe. Once we assessed the amount of care provided by nurses, we then analyzed the nursing home's vaccination record. We were optimistic to find that this nursing home vaccinated 96.40288 percent of its patients for pneumonia. To our surprise, this place was able to keep its residents out of the hospital. Indeed, it had only 1.71 hospitalizations per 1,000 long-term resident days. Unfortunately, some of its other scores in this category were not as favorable as these.
