Facility Inspections
Although we gave this place a poor overall grade, we awarded it a B+ for our inspections category. This is the result of the facility receiving a quality government inspection report this year. Our inspection scores weigh a host of factors included in a nursing home's inspection report. One key criteria we weigh heavily is the quantity and severity of deficiencies. You generally want to avoid nursing homes with a list of severe deficiencies flagged. This nursing home received 10 deficiencies on its inspection report, but none were considered to be severe. This means that CMS did not consider any of these deficiencies an immediate threat to resident health or safety. A few minor deficiencies aren't the end of the world.
Nurse Quality
One of the other reasons we rated this nursing home so highly is that it earned a very impressive nursing rating. Nursing proved to be its second best category grade. In that area, we awarded this nursing home an D. Our nursing category is based on quite a few subcategories, most of which are based on nursing hours. This nursing home provides 0.3 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Finally, we also looked at some nursing quality measures in calculating our nursing ratings. This facility performed well when it comes to avoiding pressure ulcers and major falls. We consider these metrics to be indicators of the quality of nursing care.

Long-term Care Quality
This nursing home wound up receiving a a terrible score in our long-term care area. It was given an F in this area. If you are looking for anything other than short-term rehabilitation, you should take a close look at each nursing home's long-term care scores. On top of assessing the volume of care provided by nurses aids and other staff, we analyzed the percentage of residents vaccinated against pneumonia. Fortunately, this nursing home administered the vaccine to 98.46939 percent of its patients, which is a very respectable statistic. To our surprise, this nursing home was able to limit hospitalizations. With only 0.02 hospitalizations per 1,000 long-term resident days, this nursing home had less hospitalizations than the majority of nursing homes. Unfortunately, a few of its other scores in this category were not as favorable as these.
Short-term Care Quality
The last category we graded is short-term care, where this facility was given a poor grade in this area as well. We gave it an F for this area, which is definitely very concerning. Short-term care scores are critical for people needing rehabilitation. Rehabilitation usually mandates more highly-skilled nursing services. This includes a broad scope of nursing services, ranging from registered nurses to physical and respiratory therapists, as well as other variations of therapy. As you might expect, we learned that this facility offers significantly less physical therapist and registered nurse hours per resident than the average nursing home. The last metric we assessed in this area is the number of residents who were able to leave the facility and return to the community. We discovered that just 4.2 percent of this facility's patients were able to return home, which is well below average. The combination of these poor statistics doomed this facility's short-term care score.
