Long-term Care Quality

Grade: B-minus

While we graded this nursing home extremely poorly overall, it received a more respectable long-term care grade this year. We gave it a B- in that category, which was the closest thing to a bright spot we were able to find for this facility. Long-term care ratings are based on a facility's quantity of nursing care. This includes a broad scope of personal care services, ranging from help with activities of daily living to routine medical services. On top of looking at the amount of care provided by aids and other staff, we looked at the percentage of residents vaccinated for pneumonia. This facility gave the vaccine to 100 percent of its patients, which is more than the average nursing home. This nursing home was able to keep its residents out of the hospital. It had only 0.02 hospitalizations per one thousand long-term resident days, which is an impressively low figure.

Facility Inspections

Grade: D

This nursing home received a grade of just D in the area of inspections. Unfortunately, this ended up being its second best grade. Inspection grades weigh a host of factors found on a facility's inspection report. One of the most important criteria we weigh heavily is the number and severity of deficiencies. This particular facility was assessed 4 deficiencies by CMS. The only positive thing we can say is that none of these deficiencies were in the categories that indicate that they caused a threat to resident safety or health. Finally, we also need to point out that this facility was assessed significant government fines recently of more than $50,000. This usually suggests that government inspectors became frustrated after multiple years of bad government inspections.

Nurse Quality

We gave this facility a grade of just D for our nursing rating. Our nursing grade includes many datapoints. The most important variable is the quantity of hours nurses spend with residents. This facility offered just 0.2 hours of nursing care per patient on a daily basis. This is a very low total compared to most nursing homes. Lastly, we also assessed some nursing quality-based metrics in computing our nursing scores. This facility fared well in terms of avoiding major falls and pressure ulcers.

Short-term Care Quality

Grade: D

The final area we looked at is short-term care, is this facility's weakest category. We gave this nursing home a grade of D in that category. Short-term care scores are meaningful for residents in need of rehabilitation. Rehabilitation usually mandates more skilled nursing. This includes a broad range of services, spanning from registered nurses to physical and speech therapists, as well as other variations of therapy. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The final measure we looked at in this category is the percentage of patients who were able to leave the nursing home and return to the community. We discovered that just 5.2 percent of this facility's patients were able to return home, which is well below average. The combination of these subpar metrics doomed this facility's short-term care score.
