Although we gave this facility a terrible overall grade, we awarded it a C for our inspections rating. In fact, the facility received very impressive inspection report this year. Our inspection ratings are based on many datapoints found on the a nursing home's recent inspection reports. Deficiencies are an important item to look for on these inspections. Severe deficiencies indicate a risk to resident safety. This facility was assessed 4 deficiencies on its inspection report, but none were in categories G through L. This means this facility avoided the more significant categories of deficiencies. Finally, they also received 11 substantiated complaints in recent years. This is certainly not a great sign.

This facility's next most respectable score was long-term care, but it still earned just a D in that category. This ended up being a relatively poor grade. For long-term care residents, the facility's primary objective is to maintain patients' quality of life and keep them safe. On top of considering the quantity of care provided by aids and other staff, we looked at the percentage of residents vaccinated against pneumonia. Thankfully, this facility provided the vaccine to 95.189 percent of its patients, which is an above average figure. To our surprise, this nursing home was able to limit hospitalizations. With only 1.64 hospitalizations per one thousand long-term resident days, this nursing home has less hospitalizations than the average nursing home. Unfortunately, a few of its other scores in this category weren't as favorable as these.

The third area we looked at is short-term care. This facility's was given a poor short-term care score where it was given an F. In determining our short-term care grades, we assess a facility's levels of skilled nursing, which includes registered nurses, physical therapists respiratory therapists and other licensed professionals. This category is frequently a useful assessment of the nursing home's rehabilitation. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The last datapoint we assessed in this area is the number of patients who were able to leave the facility and return home. We discovered that just 46.5 percent of this facility's residents were able to return home, which is well below average. The combination of these poor statistics sunk this facility's short-term care grade.

The final category we analyzed is nursing. Unfortunately, we gave it a lowly F for this category, which is an abysmal score. The nursing rating assesses many factors, however, the main one is the number of nurse hours spent with patients. This place offered just 3.1 hours of nursing care per resident daily. This statistic is far below average. Finally, we also looked at a few quality measures in determining our nursing ratings. This place was above average in two of the major statistics we focus on, with good marks for avoiding pressure ulcers and major falls. These statistics are generally reliable indicators of the quality of nursing care available.
