Facility Inspections
Although we gave this place a terrible overall grade, we awarded it a grade of B+ for our inspections rating. This was the result of the facility receiving a decent inspection report this year. These inspection grades take several factors into consideration, including deficiencies and federal fines. Fortunately, although this facility had a few minor dings on its government inspection report, it had zero severe deficiencies. Severe deficiencies are found in categories G, H, I, J, K and L. This means CMS did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A couple minor deficiencies are not the end of the world.
Short-term Care Quality
We also wanted to draw your attention to this facility's unfortunate short-term care score where it received a lowly F. Short-term care grades are commonly used to judge a facility's rehabilitation services. In order to have highly scored rehabilitation services, nursing homes generally need to feature better levels of skilled nursing. Skilled nursing includes registered nurses, physical therapists, respiratory therapists and other licensed professionals. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Lastly, we assessed the number of patients who were able to return to the community from this nursing home. We found that just 29 percent of this nursing home's patients returned home.

The next area we looked at was long-term care. This facility's received a poor long-term care score where it was given an F. For prospective residents in need of a permanent place to live as opposed to skilled nursing, long-term care is a very important category. One of the criteria we considered in addition to nursing hours is vaccinations. Thankfully, this facility provided the pneumonia vaccination to 94.28571 percent of its residents. Surprisingly, this place was able to limit hospitalizations. With only 1.68 hospitalizations per 1,000 long-term resident days, this facility had fewer hospitalizations than most nursing homes. Sadly, some of its other scores in this category were not as favorable as these.
Nurse Quality
The next category we scored is nursing, in which this facility received an abysmal grade in this area as well. Sadly, it received an abysmal F in this category, which is obviously very concerning. The nursing score includes a variety of data points. The most important factor is the quantity of hours nurses spend with residents.
