While we graded this facility incredibly poorly overall, it received a very impressive inspection report this year. As a result, it received one of our best grades in that category with an A. Arguably the most significant factor we look at in calculating our inspection scores is deficiencies. These deficiencies are found on a facility's recent inspection reports. It is generally best to avoid places that had a long list of deficiencies. This facility was hit with 5 deficiencies on its inspection report, but we were relieved to see that none were considered to be severe. This means that the inspectors didn't deem any of these deficiencies to cause an immediate threat to patient health or safety. A couple minor deficiencies are not necessarily the end of the world.
Long-term Care Quality
This nursing home ended up receiving a bottom of the barrel score in our long-term care area. It received a lowly F in this area. When facilities receive this kind of score in long-term care it is often a bad sign for patient care and it may indicate that the nursing home is not as well-staffed with nurses and aids. After we finished assessing the volume of care provided by nurses, we next considered the facility's vaccination statistics. Candidly, we were a bit concerned this facility vaccinated just 82.28346 percent of its residents against pneumonia. Unfortunately, this place's hospitalization rate was also a cause for concern. We found that this place had 2.47 hospitalizations per 1,000 long-term resident days, which is an alarming rate.
Short-term Care Quality
Unfortunately, this facility received an F for its short-term care score. In computing our short-term care grades, we scrutinize a nursing home's levels of highly skilled nursing, which includes registered nurses, physical therapists occupational therapists and other highly skilled individuals. This rating is frequently a solid assessment of a nursing home's rehabilitation. With its score in this category, we were not stunned to discover that this facility is well below average in terms of the number of registered nurse and physical therapy hours offered to its residents based on the figures we assessed. Honestly, this was what we expected to find. Finally, we assessed the percentage of residents that ultimately returned home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 29.6 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.

We looked at the percentage of residents sustaining pressure ulcers and major falls. This nursing home had more pressure ulcers and falls than the average facility. This could be a bad sign when you consider that many falls and bed sores are preventable with better nursing care. Unfortunately, these scores hurt this facility's nursing grade significantly.
