Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection scores account for a host of factors included in a facility's inspection report. One key criteria we consider is the quantity and severity of deficiencies. Fortunately, although this facility had some minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are those labeled as categories G through L. This means that the inspectors did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A few minor deficiencies are not necessarily the end of the world.

We gave this nursing home a grade of just D for our short-term care rating and that actually ended up being its second highest score. In our short-term care rating, we endeavor to craft a valuable barometer for rehabilitation. In this process, we look at a nursing home's offerings of skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. Not surprisingly, we found that this facility offers far less registered nurse and physical therapist hours per resident than the average facility. The last datapoint we assessed in this area is the percentage of patients who were able to leave the nursing home and return to the community. We discovered that just 42.4 percent of this facility's residents were able to return home, which is well below average. The combination of these subpar statistics doomed this facility's short-term care score.

In addition, we gave this facility an F in the area of nursing care. Nursing grades are based largely on levels of nurse staffing. This place averaged a meager 2.2 hours of nursing care per resident daily. This is a very low total compared to most nursing homes. To pair with its low totals in the area of nursing hours per resident, this place also didn't perform as well in the quality-based measures we looked at in determining our nursing scores. We looked at the percentage of residents sustaining pressure ulcers and major falls. This nursing home had more falls and pressure ulcers than the average facility.

Long-term Care Quality

Historical

The final area we graded is long-term care. We gave it a lowly F in this category, which is a rock bottom grade. When facilities receive this type of score in this category it is typically a bad sign for resident care and it may mean that the place is not as well-staffed with nurses and aids. One of the data points we considered on top of nurse's aid hours is vaccines. We were a bit alarmed this nursing home administered the pneumonia vaccination to only 76.45349 percent of its patients. To our surprise, this nursing home wasn't as bad as we expected at keeping its patients out of the hospital. While it had 2.08 hospitalizations per one thousand long-term resident days, its risk adjusted statistic was not bad due to it having more complicated patients.
