Facility Inspections
B-
Historical
Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Our inspection grades weigh several factors found on a nursing home's inspection reports. One key criteria we consider is the number and severity of deficiencies. This facility received 6 deficiencies on its inspection report, but we were relieved to see that none of the deficiencies were considered to be severe deficiencies. This indicates that the government inspectors didn't deem any of the deficiencies to create an immediate threat to resident health or safety. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Nurse Quality
B-
Historical
This nursing home really excelled in nursing, where it earned an B-. Few facilities fared better in this area. Our nursing rating is largely associated with the facility's level of nurse staffing. This nursing home provides 2.5 hours of nursing care per resident daily. This is a lower figure than we are used to seeing. Finally, we also looked at several nursing quality-based metrics. While this facility got dinged a bit for nursing hours, it actually excelled in some of these quality-based measures. With fewer than five percent of its patients suffering from pressure ulcers, it performed well in this metric. This is generally a good indicator of quality nursing care. Pressure ulcers can generally be prevented by offering better nursing care, such as having a policy of turning a patient even once per day.

Short-term Care Quality
D
Historical
Moving on to the category of short-term care, this nursing home received just a D in this area. Short-term care grades are generally employed to measure a nursing home's rehabilitation services To offer good rehabilitation services, nursing homes generally need to provide better levels of skilled nursing. Skilled nursing includes registered nurses, physical therapists, occupational therapists and other licensed professionals. With its abysmal grade in this category, we weren't stunned to find this nursing home provides far fewer registered nurse and physical therapist hours per patient than most other nursing homes. Finally, we considered the percentage of residents who were able to return to the community from this facility. This was not a strength for this nursing home. We found that just 42.8 percent of this nursing home's residents returned home. At most facilities, about half of their patients are able to return home so this is well below average.
Long-term Care Quality
F
Historical
The last category we assessed was long-term care. This facility received a very poor grade of F in this category. In a long-term care setting, the facility's primary goal is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. One of the data points we considered on top of nurse's aid hours was vaccinations. Fortunately, this facility provided the pneumonia vaccine to 99.56204 percent of its residents. Surprisingly, this facility was able to keep its patients out of the hospital. In fact, it had just 0.88 hospitalizations per 1,000 long-term resident days. Unfortunately, some of its other scores in this category weren't as favorable as these.
