While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of C. To our surprise, this place was hit with a category J through K deficiency, which are some of the worst deficiencies. As it turns out, this facility only received a D for its long-term care grade, which was actually one of its better category scores.
