Although we gave this facility a terrible overall grade, we awarded it a C for our inspections rating. In fact, the facility received very impressive inspection report this year. Our inspection scores weigh a host of factors included in a facility's inspection report. One of the most important criteria we weigh heavily is the number and severity of deficiencies. This place was assessed 7 deficiencies on its inspection report, but none were in any of the categories labeled G through L. This tells you that inspectors did not deem any of the deficiencies to present an imminent threat to patient safety or health. Unfortunately, we also need to draw your attention to the fact that this facility received some government fines in recent years.

Tragically, this facility received an F for its long-term care grade. For patients in need of a permanent place to live rather than rehabilitation, long-term care grades are an important measure. One of the datapoints we considered after nurse's aid hours is vaccinations. Thankfully, this facility provided the pneumonia vaccination to 100 percent of its patients. We were far less impressed with the nursing home's hospitalization rate. We found that this facility had 2.93 hospitalizations per 1,000 long-term resident days, which is nearly double the national average.

We also wanted to point out that this nursing home received an F for its nursing grade. When determining a facility's nursing grade, we weigh the amount of time nurses are caring for residents as well as the level of skill of those nurses. This place provided a meager 3.3 hours of nursing care per resident each day. This is an alarmingly low total. To pair with its low totals in the area of nursing hours per resident, this place also didn't perform as well in the quality-based measures we looked at in determining our nursing grades. We looked at the percentage of patients sustaining major falls and pressure ulcers. This place had more pressure ulcers and falls than the average facility.

The last category we looked at is short-term care, where this facility received an abysmal grade here as well. We gave it an F for this area, which is definitely a major disappointment. Our short-term care grade is typically employed to judge a facility's rehabilitation services. In order to have good rehabilitation services, nursing homes generally need to feature higher levels of skilled nursing. These types of services include registered nurses, physical therapists, respiratory therapists and other highly trained professionals. Considering its poor score in this category, we were not surprised to learn this facility provided substantially fewer physical therapist and registered nurse hours per patient than most other nursing homes. The final item we looked at in this category is the number of residents who were able to leave the nursing home and return to the community. We discovered that just 26.5 percent of this facility's residents were able to return home, which is well below average. The combination of these subpar statistics was damning for this nursing home's short-term care score.
