Long-term Care Quality

Grade: B-minus

Although we gave this facility a terrible overall grade, we awarded it a grade of B- for our long-term care rating. In a long-term care environment, the primary goal is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. After we finished looking at the amount of nursing care, we then analyzed the nursing home's vaccination record. We were pleased to learn that this facility vaccinates 94.347824 percent of its patients for pneumonia. Lastly, this facility was also able to limit hospitalizations. In fact, it had only 0 hospitalizations per one thousand long-term resident days, which is a remarkably low figure.

Nurse Quality

Grade: C

Remarkably, we awarded this nursing home a grade of C for our nursing rating, which isn't a bad grade. When computing a facility's nursing rating, we consider the amount of time nurses commit to residents as well as the levels of training of the nurses. This facility provides 0.3 hours of nursing care per resident each day. This is a lower figure than we are used to seeing. Lastly, nursing scores also factor in quality-based measures, such as avoiding major falls. Despite not having the most impressive total nursing hours per resident, this place performed admirably in the area of preventing major falls. This is usually an indicator that a facility has quality controls in place. Many falls can be avoided if a facility provides enough nurses aids to assist its patients.

Short-term Care Quality

Grade: D

We gave this nursing home a grade of just D for our short-term care grade. This is not a very good score. Our short-term care grades are crucial for patients requiring rehabilitation. Rehabilitation generally mandates additional skilled nursing services. Skilled nursing means a vast range of services, ranging from registered nurses to physical and respiratory therapists, as well as other forms of therapy. Considering its weak score in this area, we were not stunned to discover this place provided substantially fewer registered nurse and physical therapist hours per patient than most nursing homes. Finally, we assessed the number of patients who were able to return to the community from this facility. This definitely wasn't a strength for this nursing home. We found that just 0 percent of this nursing home's residents returned home. At most facilities, about half of their residents are able to return home so this is well below average.

Facility Inspections

Grade: D

Unfortunately, this facility did not receive a favorable inspection score. We gave it just a just D in this category. Inspection grades relate to a facility's inspection reports. We weigh several aspects of a nursing home's inspection report in calculating these inspection grades. One of those factors is health deficiencies. We recommend you scrutinize the severity of the deficiencies. This particular nursing home was hit with a category G through L deficiency, which are among the more serious deficiencies. These categories indicate that the deficiencies uncovered by inspectors had potential to cause actual harm to residents. Finally, this facility also received 3 substantiated complaints in recent years. This is yet another bad sign.
