Facility Inspections
While we did not rate this nursing home well overall, we want to point out its decent inspections in recent years. We gave it a relatively respectable grade of B- in this category. Our inspection ratings are tied to many datapoints found on the a nursing home's recent inspection reports. Deficiencies are a key item to look for on these reports. Severe deficiencies indicate a threat to patient safety. This facility received 5 deficiencies on its inspection report, but fortunately none of the deficiencies were considered to be severe. This indicates that the inspectors didn't consider any of the deficiencies to be an immediate risk to patient health or safety. Finally, CMS cited this nursing home for possible abuse or neglect. Hopefully, its performance improves in the future, but until then we simply can't recommend this facility.
Nurse Quality
We also ended up giving this facility a fairly respectable grade in the area of nursing, where it received a grade of B-. Our nursing score is largely associated with the nursing home's level of nurse staffing. This facility averages just 0.4 hours of nursing care per resident daily, which is a very low figure. Despite having low quantities of nursing care, this place actually excelled in some of the quality measures we assessed. It performed well when it comes to minimizing its residents' major falls and pressure ulcers.

Long-term Care Quality
The next area we graded is long-term care, where this facility received a grade of C in this area. Long-term care grades are typically used to grade a facility's performance as a traditional convalescent home, as opposed to focusing more on the skilled nursing services provided by a rehabilitation facility. Once we looked at the amount of care provided by nurses and other staff, we then analyzed the nursing home's vaccination record. We were optimistic to discover that this nursing home vaccinated 100 percent of its patients for pneumonia. This place was able to keep its patients out of the hospital. Indeed, it had only 0.01 hospitalizations per 1,000 long-term resident days, which is an impressively low number.
Short-term Care Quality
The last category we assessed was short-term care, turned out to be this nursing home's worst area. We awarded this nursing home a D in this area. In determining our short-term care grades, we size up a facility's levels of skilled nursing, which includes registered nurses, physical therapists occupational therapists and other licensed professionals. This score is typically a useful assessment of a nursing home's rehabilitation services. Considering its score in this area, we weren't surprised to find that this nursing home is well below average in terms of the number of physical therapy and registered nurse hours offered to its patients based on the metrics we looked at. Honestly, this was about what we expected here. Lastly, we considered the number of residents who who were able to eventually return home from this facility. This nursing home didn't fare well here either. In fact, we found that just 4 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.
