One of the many reasons this turned out to be a great nursing home is that it received an A+ long-term care grade. This is simply as good as it gets in this category. Nursing homes that receive this type of grade in this category tend to provide consistent 24/7 care to make sure residents are kept in good health. On top of very favorable nursing hour statistics, this facility's vaccination records was as good as it gets also. Indeed, it vaccinated 100 percent of its patients for pneumonia. The last statistic we looked at was the nursing home's hospitalization rate. Here we found that this place had 3.11 hospitalizations per one thousand long-term resident days. While this wasn't as favorable as most of its other scores in this category, this statistic may be skewed for some nursing homes based on some of the medical complexity of patients.

Adding to an already impressive profile, this facility also received virtually flawless health inspections in recent years. As a result, we awarded it an A+ inspection grade. Perhaps the most significant factor we look at in computing our inspection scores is deficiencies. Deficiencies are found on a nursing home's inspection reports. Places with better scores in this area typically dodged the more severe deficiencies involving things like patient abuse. While this place had some minor dings on its government inspection report, it had zero severe deficiencies. Severe deficiencies are those labeled as categories G, H, I, J, K and L. This means that CMS didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

The third category we looked at was short-term care. This facility also was awarded an A+ short-term care grade, making it one of the few facilities to receive multiple A+ category grades. In our short-term care rating, we seek to craft a valuable gauge for rehabilitation. In this process, we assess the facility's levels of skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. This facility provides more services with registered nurses and physical therapists than most facilities we looked at. Finally, we looked at the number of patients that were able to return home from this facility. In addition to excelling in the area of physical therapy, we found that it fared as well as just about any facility in this area with 66.2 percent of its patients returning home. Most facilities are below 50 percent in this statistic.

Moving on to our final category, this facility also performed admirably in our nursing category. In fact, we gave it an A in that category. This wrapped up truly elite profile. Our nursing score consists of several components, most of which are associated with quantities of nurse staffing. This particular nursing home provided 4.2 hours of nursing care per resident per day, which was among the highest totals we found. Lastly, this facility also excelled in several quality measures we assessed. With less than five percent of its residents suffering from pressure ulcers, this place performed as well as any nursing home the nation in this category. This is often a good indicator that a facility has reliable quality controls. Many pressure ulcers can be avoided by offering better nursing care and having a protocol of turning residents even once per day.
