Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a C for our inspections rating. In fact, the facility received very impressive inspection report this year. Our inspection grades weigh a host of factors found on a facility's inspection reports. One key criteria we weigh heavily is the number and severity of deficiencies. This place had 2 deficiencies on its government inspection report, but none were in any of the categories labeled G through L. This indicates the government inspectors did not deem any of the deficiencies to be an imminent risk to patient health or safety. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.
Nurse Quality
We gave this facility a grade of just D for our nursing rating and that actually turned out to be its second highest score. Our nursing score is primarily associated with the facility's level of nurse staffing. This place offered only 0.3 hours of nursing care per patient each day. This is a very low total compared to most nursing homes. Lastly, we also assessed some nursing quality-based metrics in calculating our nursing scores. This facility performed well in terms of minimizing its residents' pressure ulcers and major falls. We consider these statistics to be reliable measures of the quality of nursing care.

Short-term Care Quality
The third category we graded was short-term care, in which this facility was given an F. In determining our short-term care scores, we size up the nursing home's levels of highly skilled nursing services, which includes registered nurses, physical therapists respiratory therapists and other highly trained professionals. This category is more often than not a meaningful assessment of a facility's rehabilitation services. As you might expect, we found that this place provides substantially less registered nurse and physical therapist hours per patient than the typical facility. The last metric we assessed in this category is the number of residents who were able to leave the facility and return to the community. We discovered that just 4.4 percent of this facility's residents were able to return home, which is well below average. The combination of these poor metrics doomed this nursing home's short-term care score.
Long-term Care Quality
The last category we rated was long-term care. This nursing home received a rock bottom grade of F in that category. Nursing homes that receive this kind of grade in long-term care likely don't provide the kind of consistent 24/7 care that some other facilities offer. In addition to assessing the volume of care provided by nurses and other staff, we looked at the number of residents vaccinated against pneumonia. We were a bit concerned this nursing home administered the vaccine to only 86.94639 percent of its residents. To our surprise, this facility was actually decent at keeping its residents out of the hospital. It had just 0.03 hospitalizations per one thousand long-term resident days. This is its best score in this category.
