Short-term Care Quality
Even though this place's overall grade was as bad as it gets, it actually didn't perform terribly in our short-term care rating. In fact, we awarded it a grade of B- for that category, which is a decent score. Unfortunately, this was not enough to salvage its profile. Our short-term care scores are based in part on the facility's quantity of skilled highly skilled professionals. This means a wide range of services, spanning from registered nurses to physical and occupational therapists, as well as other types of therapy. In this nursing home's case, we were surprised to learn that it actually provides less physical therapist and registered nurse hours per resident than a typical facility. The hope is that this does not adversely impact the quality of care. The last item we assessed in this category is the percentage of patients that ultimately returned home from the nursing home. We found that just 5 percent of this nursing home's patients returned home.
Facility Inspections
This facility did not receive favorable inspection reports in recent years. In fact, it received a grade of just D for its inspection rating, which was unfortunately its second best category. Inspection ratings account for several factors included in a facility's inspection reports. One key criteria we look at is the quantity and severity of deficiencies. This particular facility was hit with 6 deficiencies by government inspectors. The only favorable thing we can say is that none of these deficiencies were in the categories that suggest they created a risk to resident health or safety. Lastly, we need to point out that this facility was assessed large government fines recently of more than $100,000. This usually means that a facility'sbad track record spanned more than one year.

Nurse Quality
Among this nursing home's several poor grades was its F in the area of nursing care. Our nursing score features several subcategories. The most heavily weighted one is the number of hours nurses spent with residents. This facility averaged only 0.2 hours of nursing care per patient on a daily basis. This is a very low total compared to most nursing homes. Lastly, we also looked at some nursing quality measures in calculating our nursing grades. This nursing home performed well when it comes to minimizing its residents' pressure ulcers and major falls.
Long-term Care Quality
The next area we assessed is long-term care. Sadly, it received an F in this area, which is an abysmal grade. For prospective residents in need of a permanent place to live rather than skilled nursing, long-term care grades are a key measure. On top of assessing the volume of care provided by nurses aids and other staff, we looked at the percentage of residents vaccinated against pneumonia. This nursing home administered the vaccine to 94.117645 percent of its patients. Surprisingly, this facility was able to limit hospitalizations. With just 0.02 hospitalizations per one thousand long-term resident days, this nursing home has less hospitalizations than most nursing homes. Unfortunately, a few of its other scores in the category were not as favorable as these.
