Facility Inspections
On top of performing well overall, this facility also received nearly flawless government inspections. As a result, it received one of our best grades in that category with an A+. Our inspection grades weigh a host of factors found on a facility's inspection report. One of the most important criteria we weigh heavily is the quantity and severity of deficiencies. Facilities with better scores in this category typically have very few severe deficiencies. Although this nursing home had a few deficiencies on its inspection report, none of them were serious based on CMS' deficiency scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Long-term Care Quality
We also gave this nursing home a score of A- for our long-term care grade. Long-term care scores of this caliber generally require both 24/7 care from nurses and aids, as well as quality routine medical care. One of the criteria we considered in addition to nursing hours is vaccines. This facility administered the pneumonia vaccination to 100 percent of its residents. Vaccination is a great way to minimize negative health outcomes for the senior population. Clearly, this nursing home is doing something right in this area as it also excelled at keeping its patients out of the hospital. It had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low figure.

Short-term Care Quality
This facility really excelled in the area of short-term care, where it received a grade of A-. Very few facilities performed more favorably in this category. In our short-term care score, we seek to create a valuable measure for rehabilitation. In doing so, we analyze a nursing home's levels of skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. Fortunately, it looks like this nursing home has registered nurses on staff. Not every facility employs these types of nurses. However, based on the data this nursing home provided, they do not appear to employ physical therapists. The last metric we considered in this area is the number of residents who ultimately returned home from the nursing home. We found that 5.3 percent of this nursing home's patients returned home as opposed to remaining at the facility on a permanent basis.
Nurse Quality
The final category we graded is nursing, which is this facility's least impressive category. We awarded this facility a D in that area. However, you should not judge this nursing home solely on one weak category, since it had a few high-end scores in other areas. Our nursing rating features several data points. The most heavily weighted one is the amount of time nurses spend with residents. This nursing home provided just 0.4 hours of nursing care per resident per day. Unfortunately, this place also didn't fare as well as we had hoped in some of the quality-based metrics we looked at for this category.
