Facility Inspections
While this nursing home's overall grade was as bad as it gets, it actually excelled in the category of inspections. In fact, we awarded it a grade of A for that category, which is one of our highest scores. Our inspection ratings weigh several factors found on a nursing home's inspection reports. One of the most important criteria we rely on is the quantity and severity of deficiencies. You should avoid places with a list of severe deficiencies flagged. This place was hit with 2 deficiencies on its inspection report, but fortunately none were considered to be major deficiencies. This indicates that CMS didn't deem any of the deficiencies to create an immediate risk to resident safety or health. A few minor deficiencies are not the end of the world.
Long-term Care Quality
Unfortunately, we gave this facility an F for its long-term care grade. In a long-term care environment, the facility's primary goal is to maintain patients' quality of life and keep them safe. In addition to considering the volume of care provided by nurses aids and other staff, we analyzed the percentage of residents vaccinated against pneumonia. This nursing home vaccinated 97.85714 percent of its patients, which is higher than the majority of nursing homes. Surprisingly, this facility also fared well at keeping its residents out of the hospital. It had only 1.54 hospitalizations per one thousand long-term resident days. Sadly, a few of its other scores in the category weren't as favorable as these.

Nurse Quality
Among this nursing home's several poor grades was its F in the area of nursing care. Our nursing rating focuses on the facility's nurse staffing levels. We look at both the levels of skill of the nurses and the number of hours spent with patients. This facility provided a meager 3.1 hours of nursing care per resident daily. To pair with its low totals in the area of nursing hours per patient, this nursing home also didn't fare as well in the quality-based measures we looked at in determining our nursing scores. We looked at the percentage of residents experiencing pressure ulcers and major falls. This nursing home had more pressure ulcers and falls than the average facility.
Short-term Care Quality
Turning to our fourth area, this facility really did not fare very well in this category either. With a bottom of the barrel grade of F in short-term care, this is just about as bad as it gets. In crafting these short-term care ratings, we analyze the nursing home's skilled nursing services, such as those performed by registered nurses, respiratory therapists, physical therapists and other types of therapists. Our goal is to devise a measure for comparing the rehabilitation services of nursing homes. Not surprisingly, we discovered that this place provides significantly less physical therapist and registered nurse hours per resident than the typical nursing home. The final datapoint we assessed in this area is the number of patients who were able to leave the facility and return to the community. We discovered that just 39 percent of this facility's patients were able to return home, which is well below average. The combination of these poor metrics was damning for this nursing home's short-term care score.
