Facility Inspections
While this nursing home's overall grade was terrible, it performed well in the category of inspections. In fact, we awarded it a B for that area, which is one of our better scores. Our inspection ratings are tied to many items found on the a nursing home's recent inspection reports. Deficiencies are an important item to look for on these reports. You should especially avoid facilities with severe deficiencies linked to endangerment of patients. Although this nursing home had some deficiencies on its report, none of them were severe based on CMS' deficiency scale. A few minor deficiencies aren't necessarily the end of the world.
Long-term Care Quality
Turning to the area of long-term care, this nursing home was given a grade of just D in that area. Unfortunately, this ended up being one of its better features. When nursing homes receive a score in this range in this category it generally means the facility did not perform well in our quality measures relating to patient care. One of the datapoints we considered on top of nursing hours was vaccinations. We were a bit alarmed this nursing home provided the pneumonia vaccination to only 35.840706 percent of its patients. Surprisingly, this facility was actually able to limit hospitalizations. With only 0.03 hospitalizations per 1,000 long-term resident days, this facility has less hospitalizations than the majority of nursing homes. This is its best feature in this category.

Nurse Quality
Unfortunately, we gave this facility an F for its nursing grade. In determining our nursing scores, we weigh both staffing levels and the skill levels of the nurses. This facility provided only 0.3 hours of nursing care per resident each day. Finally, we also assessed some nursing quality measures in calculating our nursing ratings. This place fared well in terms of avoiding major falls and pressure ulcers.
Short-term Care Quality
The final area we graded was short-term care. This nursing home received a very poor grade of F in this area. Short-term care grades are generally employed to measure a facility's rehabilitation services. In order to provide highly graded rehabilitation services, facilities generally must feature better levels of highly skilled nursing services. These types of services include registered nurses, physical therapists, respiratory therapists and other highly skilled individuals. Not surprisingly, we found that this place provides significantly less physical therapist and registered nurse hours per resident than the typical facility. The last item we assessed in this area is the percentage of patients that returned home from the nursing home. This nursing home struggled quite a bit in this area as well, with just 3.5 percent of its residents returning home. Unfortunately, this was well below average.
