Facility Inspections
While we didn't give this facility a good overall score, we actually gave it a strong grade in the area of inspections, where it received a B+. Inspection scores are tied to many pieces of information found on the a nursing home's recent inspection reports. Deficiencies are an important item to look for on these inspections. You should especially avoid nursing homes with severe deficiencies linked to endangerment of patients. This place received 4 deficiencies on its inspection report, but fortunately none of the deficiencies were considered severe. This indicates that CMS did not consider any of the deficiencies to pose an immediate risk to resident health or safety. A few minor deficiencies aren't the end of the world.
Long-term Care Quality
We also gave this facility a decent grade in the category of long-term care, with a grade of C. In determining our long-term care scores, we assess the personal care received by a nursing home's patients. In addition to looking at the quantity of care provided by nurses aids and other staff, we analyzed the number of residents vaccinated for pneumonia. Candidly, we were a bit worried that this facility vaccinated just 87.5 percent of its residents. Nevertheless, this place keeps its residents out of the hospital. Indeed, it had only 1.49 hospitalizations per 1,000 long-term resident days.

Nurse Quality
We were surprised to find that this facility also excelled in the area of nursing. We awarded it an D for that category, which is one of our highest scores. Our nursing rating includes several subcategories. The most important one is the number of hours nurses spent with residents. With only 2.9 hours of nursing care per patient per day, this place's nurse staffing levels were quite a bit below the national average. Lastly, we also looked at a few quality measures in computing our nursing grades. This place was above average in two of the major areas we focus on, with good scores for minimizing its residents' pressure ulcers and major falls.
Short-term Care Quality
The next category we graded was short-term care. This nursing home was given a bottom of the barrel grade of F in this area. In the area of short-term care, we attempt to evaluate measures of a nursing home's rehabilitation. We analyze the nursing home's skilled nursing services, such as the ones performed by registered nurses and physical, occupational and other therapists. With a score in this range for this area, we weren't shocked to discover that this nursing home was well below average in many staffing areas we assess. The facility supplied significantly fewer registered nurse and physical therapist hours per resident than most other nursing homes. The final item we assessed in this area is the percentage of patients that who were able to eventually return home from the facility. This facility struggled quite a bit in this area as well, with just 44.6 percent of its patients returning home.
