Facility Inspections

Grade: B

While this facility's overall score was as bad as it gets, it fared well in the category of inspections. In fact, we awarded it a grade of B for that category, which is one of our better scores. Perhaps the most critical factor we look at in calculating our inspection scores is deficiencies. These deficiencies are found on a facility's inspection reports. It is generally in your best interest to avoid facilities that have a long list of severe deficiencies. This place was hit with 6 deficiencies on its inspection report, but fortunately none were considered to be major deficiencies. This indicates that CMS did not deem any of these deficiencies to cause an immediate threat to resident health or safety. A couple minor deficiencies are not necessarily the end of the world.

Long-term Care Quality

Grade: C

This facility also didn't fare poorly in the category of long-term care. In fact, we awarded it a C in that category, which turned out to be another one of its higher grades. In computing our long-term care scores, we assess the assistance with daily living received by a nursing home's residents. In addition to looking at the quantity of care provided by nurses and other staff, we also looked at the number of residents vaccinated for pneumonia. This nursing home gave the vaccine to 100 percent of its patients, which is higher than most nursing homes. Lastly, this facility was also able to limit hospitalizations. In fact, it had less than one hospitalization per one thousand long-term resident days, which is an impressively low number.

Nurse Quality

Grade: D

We were surprised to find that this nursing home also excelled in nursing. We gave it a score of D for that category, which is one of our highest scores. In calculating a nursing home's nursing score, we weigh the amount of time nurses commit to patients and the level of licensure of the nurses. This place provided a meager 0.3 hours of nursing care per patient per day. This is an alarmingly low total. Finally, we also looked at some nursing quality-based metrics in calculating our nursing ratings. This place performed well in the area of avoiding major falls and pressure ulcers.

Short-term Care Quality

Grade: F

The last category we graded is short-term care. We gave it a lowly F for this area, which is a rock bottom score. In computing these short-term care scores, we quantify the facility's skilled nursing services, including those performed by registered nurses, speech therapists, physical therapists and other types of therapists. The objective is to create a tool for sizing up the rehabilitation services of nursing homes. With its score in this area, we weren't surprised to learn that this place was well below average in terms of the quantity of physical therapy and registered nurse hours provided to its residents based on the data we assessed. The final metric we looked at in this area is the number of residents that ultimately were able to return home from the nursing home. This facility struggled quite a bit in this metric as well, with just 4.3 percent of its residents returning home. This was well below the national average.
