Facility Inspections
While we were not high on this nursing home's overall grade, it received a very impressive inspection report this year. In fact, it received a nearly flawless inspection report this year. As a result, it earned one of our highest grades in that category with a grade of A-. Inspection grades are based on several items located in the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these inspections. Most importantly, you should avoid facilities with severe deficiencies associated with endangerment of patients. Fortunately, although this facility had a few minor dings on its inspection report, it had zero severe deficiencies. Severe deficiencies are those found in categories G, H, I, J, K and L. This tells you that the inspectors did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies aren't the end of the world.
Short-term Care Quality
One of this facility's higher category scores was in the category of short-term care. This turned out to be its second most favorable category. In that category, we gave this facility a C. With our short-term care grade, we endeavor to craft a meaningful barometer for rehabilitation services. In this process, we assess the facility's level of highly-skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. This nursing home boosted its score in this category by offering more physical therapy hours to its residents than the average facility. The last datapoint we looked at in this area is the percentage of patients who were able to leave the facility and return to the community. We found that it was above average in this metric with 54.7 percent of its patients returning home.

Nurse Quality
We were surprised to find that this nursing home also excelled in the category of nursing. We awarded it an impressive grade of D for that category, which is one of our best grades. Our nursing grade is based on several components, many of which are based on nursing hours. This place provided only 2.2 hours of nursing care per resident each day. Finally, we also looked at some quality measures in computing our nursing ratings. This facility was above average in two of the major datapoints we considered, with good marks for avoiding major falls and pressure ulcers.
Long-term Care Quality
The next category we graded was long-term care. We gave it an abysmal F in this category, which is an abysmal score. When nursing homes receive a grade in this range in long-term care it typically means the nursing home did not perform well in our quality measures relating to patient care. On top of considering the amount of care provided by nurses and other staff, we analyzed the percentage of residents vaccinated against pneumonia. We discovered that this nursing home vaccinated just 82.35294 percent of its patients, which is somewhat alarming. To our surprise, this nursing home was actually decent at keeping its patients out of the hospital. It had only 0 hospitalizations per 1,000 long-term resident days. This is its best score in this category.
