Facility Inspections

Grade: B-minus

Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. We believe that one of the most critical factors in evaluating an inspection report is deficiencies. We especially look at the severity of the deficiencies. This facility was hit with 4 deficiencies on its inspection report, but fortunately none of the deficiencies were considered to be severe deficiencies. This tells you that the inspectors didn't consider any of the deficiencies an imminent risk to patient health or safety. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.

Long-term Care Quality

Grade: B-minus

We were also surprised to discover that this facility earned a somewhat respectable long-term care rating in our assessment. We gave it a B- in that category. In a long-term care environment, the primary goal is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. In addition to considering the amount of care provided by nurses aids and other staff, we also looked at the number of residents vaccinated against pneumonia. This nursing home vaccinated 100 percent of its patients, which is more than the majority of nursing homes. Finally, this nursing home was also able to limit hospitalizations. Indeed, it had only 0.01 hospitalizations per 1,000 long-term resident days, which is a remarkably low figure.

Nurse Quality

Grade: D

We gave this facility a grade of just D for our nursing rating. The nursing rating includes many factors. The most heavily weighted factor is the amount of time nurses spent with patients. With only 0.3 hours of nursing care per patient per day, this nursing home's nurse staffing levels were well below the national average. Lastly, we also looked at some nursing quality-based metrics in calculating our nursing scores. This nursing home fared well when it comes to minimizing its residents' major falls and pressure ulcers. We consider these areas to be good indicators of the quality of nursing care.

Short-term Care Quality

Grade: D

The final category we graded is short-term care, is this facility's worst area. We gave this facility a grade of D in this area. Our short-term care ratings are important for prospective residents seeking rehabilitation. Rehabilitation usually mandates additional skilled nursing services. This includes a wide spectrum of services, ranging from registered nurses to physical and speech therapists, as well as other types of therapy. Given its score in this category, we were not surprised to find that this place was well below average in terms of its number of registered nurse and physical therapy hours offered to its residents based on the measures we assessed. Frankly, this was about what we expected here. The final item we considered in this category is the percentage of residents that eventually returned home from the facility. This nursing home struggled quite a bit in this metric as well, with just 3.8 percent of its residents returning home. This was quite a bit below the national average.
