Nurse Quality
C
Historical
Even though this facility's overall grade was terrible, it didn't perform as badly in the area of nursing. We awarded it a grade of C for that area. We assessed the qualifications of nurses at the nursing home, in addition to the quantity of time those nurses spent with residents, in computing our rating in this category. This nursing home provides 3.6 hours of nursing care per resident per day. Finally, we also considered some quality-based assessments. This facility performed well in some of the quality measures we looked at. In terms of the percentage of its residents suffering from pressure ulcers, this facility beat the national average.
Long-term Care Quality
D
Historical
We awarded this facility a grade of just D for our long-term care score and that actually ended up being its second most respectable category. For patients in need of a permanent residence as opposed to rehabilitation, long-term care is an important category. Once we assessed the quantity of care provided by aids and other staff, we turned to the facility's vaccination statistics. This facility vaccinated 100 percent of its patients for pneumonia, which is more than the vast majority of nursing homes. We were far less impressed with the facility's hospitalization rate. We found that this nursing home had 3.39 hospitalizations per 1,000 long-term resident days, which is nearly twice the national average.

Short-term Care Quality
D
Historical
Sadly, this facility's next highest category was short-term care. As it turns out, with a grade of D in this area, it still fared relatively badly. Short-term care ratings are based in part on the nursing home's quantity of skilled highly skilled professionals. This means a vast scope of services, ranging from registered nurses to physical and speech therapists, not to mention other types of therapy. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Lastly, we assessed the number of residents that who were able to eventually return home from this nursing home. This facility didn't fare well here either. We found that just 48.3 percent of this facility's patients were able to return home. This figure was below the national average.
Facility Inspections
F
Historical
Turning to our final category of inspections, this facility received a grade that's as bad as it gets. We weight this category more heavily than our other categories. When a nursing home has this poor of an inspection rating, you should expect some severe deficiencies on its inspection report. Severe deficiencies indicate a risk to the well being of residents. This particular facility was assessed one category L deficiency, which is the most severe category of deficiencies. This category indicates inspectors found a widespread deficiency putting resident safety or health in imminent jeopardy. Even a single deficiency in this category can drag a facility's inspection score down. We are also concerned by the fact that this facility was flagged by CMS for possible abuse or neglect. We prefer to avoid facilities with this designation. Please do your best to find an alternative nursing home.
