Facility Inspections
Although we gave this nursing home a terrible overall grade, we gave it an A- for our inspections rating. This grade is far more impressive than the facility's overall grade. Our inspection ratings are tied to several pieces of information found on the a nursing home's recent inspection reports. Deficiencies are a key item to look for on these inspections. You should especially avoid nursing homes that have severe deficiencies associated with risks to resident well being. This place received 2 deficiencies on its inspection report, but we were relieved to see that none were considered to be major deficiencies. This means that CMS did not consider any of these deficiencies to create an imminent threat to patient health or safety. A couple minor deficiencies are not necessarily the end of the world.
Long-term Care Quality
We awarded this nursing home a grade of just D for our long-term care rating and that actually turned out to be its next best category. If you are looking for anything other than short-term rehabilitation, you should take a close look at each facility's long-term care scores. After considering the volume of care provided by aids and other staff, we then analyzed the nursing home's vaccination record. We were optimistic to learn that this nursing home vaccinated 94.84849 percent of its residents against pneumonia. To our surprise, this nursing home was able to limit hospitalizations. With only 0.02 hospitalizations per one thousand long-term resident days, this nursing home had fewer hospitalizations than the majority of nursing homes. Unfortunately, some of its other scores in the category weren't as favorable as these.

Short-term Care Quality
We awarded this nursing home just a D for our short-term care rating. This is not a very good score. In forming our short-term care grades, we quantify the nursing home's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. Our goal is to formulate a barometer for comparing the rehabilitation services of different nursing homes. Given its score in this category, we weren't stunned to find that this nursing home is well below average in terms of the quantity of physical therapy and registered nurse hours provided to its residents based on the metrics we assessed. Lastly, we considered the number of residents who returned home from this nursing home. This facility didn't fare well here either. In fact, we found that just 0 percent of this facility's patients were able to return home.
Nurse Quality
The final area we graded was nursing. This nursing home received an abysmal grade of F in this category. We looked at the qualifications of nurses employed by the nursing home, in addition to the number of hours those nurses were with patients, in calculating our score in this area. With a meager 0.3 hours of nursing care per resident on a daily basis, this nursing home's nurse staffing levels were far below the national average. To pair with its low totals in the area of nursing hours per resident, this facility also didn't fare as well in the quality-based measures we looked at in determining our nursing scores. We looked at the percent of patients experiencing pressure ulcers and major falls. This facility had more pressure ulcers and falls than the average facility. This could be a bad sign when you consider that many falls and bed sores are preventable with better nursing care. Unfortunately, these metrics pulled down this nursing home's nursing rating significantly.
