While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of C. We believe that one of the most important factors in assessing an inspection report is deficiencies. We especially look at the severity of the deficiencies. Surprisingly, this facility was hit with a category J through K deficiency, which are among the worst possible categories of deficiencies. This typically tells you inspectors found a deficiency which places patient safety or health in imminent danger. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.

Remarkably, we gave this facility a C for its short-term care grade, which is not a bad grade. In the area of short-term care, we seek to assess measures of a nursing home's rehabilitation. We analyze the facility's skilled nursing services, including those performed by registered nurses and physical therapists. One of this nursing home's strengths is physical therapy hours. We found that it offers at least 50% more hours with physical therapists to its residents than most other facilities. Finally, we looked at the number of residents who were able to return to the community from this nursing home. It performed decently in this area, with 45.6 percent of its residents returning home.

Among this nursing home's several poor grades was its F in the area of nursing care. The nursing category is based on quite a few datapoints, many of which are tied to nursing hours. This nursing home provides 3.1 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. This nursing home also had abysmal scores in some of the quality-based measures to pair with its low nursing hour totals. For starters, we looked at the percent of patients sustaining pressure ulcers. This nursing home was at more than twice the national average in this datapoint. This is concerning when you consider that many bed sores are believed to be preventable with better nursing care. This ended up sealing the facility's fate of receiving a very poor nursing score.

Switching gears to the final area, this facility really didn't perform very well in this area either. With a very poor grade of F in long-term care, this is about as bad as it gets. Nursing homes that do not fare well in this category often don't provide as much nursing care and also may be struggling in some of the areas of routine personal care we assessed. On top of considering the volume of care provided by nurses aids and other staff, we also looked at the percentage of residents vaccinated against pneumonia. This nursing home gave the vaccine to 93.76947 percent of its residents. This is a bit less than we expected but still a decent percentage. The last statistic we looked at is its hospitalization rate. We found that this facility had 1.78 hospitalizations per one thousand long-term resident days. This is a higher hospitalization rate than the average nursing home, but it is not quite as bad as we feared it might be.
