This facilities grades were terrible across the board. In particular, we wanted to draw your attention to its poor long-term care grade where it received an F. When nursing homes receive a grade in this range in long-term care it generally means the nursing home did not perform well in our quality measures relating to patient care. On top of assessing the amount of care provided by aids and other staff, we also looked at the number of residents vaccinated against pneumonia. This nursing home provided the vaccine to 90.3125 percent of its residents. This is significantly below average. Lastly, we looked at its number of hospitalizations. With 1.87 hospitalizations per 1,000 long-term resident days, this facility didn't fare as well as the average nursing home.

This nursing home wound up receiving a a terrible grade in our short-term care category. It was given a lowly F in this area. Our short-term care scores are likely more critical for individuals needing a nursing home for rehabilitation. Rehabilitation typically mandates higher levels of skilled nursing services. Skilled nursing means not just nursing, but also physical and respiratory therapy, as well as other variations of therapy. Given its poor grade, we were not shocked to find that this nursing home provided substantially fewer registered nurse hours per resident than most other facilities. Finally, we considered the number of residents that eventually returned home from this facility. This facility didn't fare well in this area. In fact, we found that just 25.9 percent of this facility's patients were able to return home. This figure was quite a bit off the national average.

We also wanted to draw your attention to this facility's poor nursing grade where it received an F. Our nursing score is based on the nursing home's nurse staffing levels. We weigh both the levels of training of those nurses as well as the number of hours spent with patients. This nursing home provides 3.8 hours of nursing care per resident per day, which is slightly below the national average. In addition to being below average in nursing hours per resident, this nursing home also had poor marks in the quality-based measures we looked at in computing our nursing scores. We looked at the percent of residents sustaining pressure ulcers and major falls. This facility was at roughly 1.5 times the national average in both of these metrics. This is likely a bad sign when you consider that many falls and bed sores are preventable with better nursing care. These metrics pulled down this nursing home's nursing grade significantly.

In our last category of inspections, this facility received an F in this area. For nursing homes with poor inspection scores, you should focus on any severe deficiencies found on their inspection reports. This particular facility was assessed 4 deficiencies by CMS. The only favorable thing we found is that none of the deficiencies were in the categories that suggest that they created a risk to resident health or safety. Lastly, we also want to point out that this place was assessed large government fines amounting to over $100,000. This typically suggests that a nursing home's bad performance spanned more than one year.
