Facility Inspections
To pair with its relatively modest overall score, we awarded this facility an A for its inspection grade. This grade is far better than the facility's overall grade. Arguably the most significant factor we consider in determining our inspection scores is deficiencies. These deficiencies can be found on a nursing home's recent inspection reports. Facilities with better grades in this area typically dodged the most severe deficiencies involving things like patient abuse. While this facility had a few deficiencies on its report, none were serious based on CMS' scale. A couple of minor deficiencies shouldn't lead you to cross a nursing home off your list.
Long-term Care Quality
We also awarded this nursing home a solid grade in the category of long-term care, where it received a B-. For residents looking for a permanent place to live as opposed to skilled nursing, long-term care grades are an important measure. In addition to considering the volume of care provided by nurses and other staff, we also looked at the percentage of residents vaccinated against pneumonia. Fortunately, this facility administered the vaccine to 95.19451 percent of its residents, which is a very respectable figure. This place keeps its residents out of the hospital. It had only 0.01 hospitalizations per one thousand long-term resident days, which is an impressively low number.

Short-term Care Quality
Unfortunately, this nursing home only received a grade of D for its short-term care grade, which is not one of our better scores. Short-term care grades are critical for people requiring rehabilitation. Rehabilitation generally requires additional skilled nursing. This includes a wide range of nursing services, ranging from registered nurses to physical and speech therapists, as well as other variations of therapy. Not surprisingly, we discovered that this nursing home offers far less registered nurse and physical therapist hours per patient than the average facility. Finally, we considered the percentage of residents who eventually returned home from this nursing home. This facility didn't fare well here either. In fact, we found that just 5.2 percent of this facility's residents were able to return home.
Nurse Quality
This facility's worst area was nursing, which is the last category we assessed. We gave it a lowly F for this area. Even with some acceptable scores in other categories, this score is nevertheless disappointing so we would like to to make a note of it. Our nursing grade consists of many data points, but the most important one is the level of nurse hours per patient per week. This place averaged a meager 0.3 hours of nursing care per resident each day. In addition to its weak nursing hour totals, this place received poor marks in some of the quality-based metrics we looked at in calculating our nursing scores. We found that a higher percentage of this nursing home's patients sustained falls which led to major injury. We believe that many falls could be avoided with better nursing care. This metric contributed to this facility receiving a poor nursing score.
