Facility Inspections
B-
Historical
Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection ratings weigh several factors found on a nursing home's inspection reports. One key criteria we look at is the quantity and severity of deficiencies. This nursing home received 10 deficiencies on its inspection report, but fortunately none were considered to be major deficiencies. This tells you that the inspectors didn't consider any of the deficiencies to create an immediate risk to patient health or safety. Lastly, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
Nurse Quality
C
Historical
We were also surprised to find that this facility received a decent nursing grade this year. We gave it a grade of C in that category. Our nursing rating analyzes the nursing home's nurse staffing levels. We look at both the levels of training of the nurses as well as the quantity of hours spent with residents. Based on the CMS data, this nursing home offers 3.8 hours of nursing care per resident per day. Finally, our nursing grades also factor in quality-based metrics, such as minimizing major falls. This place performed admirably in this subcategory. Avoiding serious falls is usually a reliable indicator that a nursing home has quality controls in place. Many falls can be avoided if a nursing home offers enough nurses to assist its patients.

Short-term Care Quality
D
Historical
We gave this facility just a D for our short-term care grade. This is well below average in this category. In our short-term care assessment, we attempt to forge a fair gauge for rehabilitation services. In this process, we analyze the nursing home's levels of highly-skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. In this nursing home's case, we found that it provides less physical therapist and registered nurse hours per resident than the average facility. Lastly, we considered the percentage of patients that eventually returned home from this facility. This facility didn't fare well here either. In fact, we found that just 42.2 percent of this facility's patients were able to return home.
Long-term Care Quality
F
Historical
The next area we looked at is long-term care. It received an abysmal F for this category, which is a bottom of the barrel grade. For residents in need of a permanent place to live rather than rehabilitation, long-term care grades are very important. Once we looked at the quantity of care provided by nurses aids and other staff, we next considered the facility's vaccination record. We were optimistic to find that this nursing home vaccinated 94.86607 percent of its patients against pneumonia. Surprisingly, this facility also keeps its patients out of the hospital. In fact, it had just 1.5 hospitalizations per one thousand long-term resident days. Sadly, a few of its other scores in this category were not as favorable as these.
