Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. These inspection scores take several factors into consideration, including deficiencies and federal fines. You can learn more about each of these factors by obtaining copies of nursing homes' inspection reports. While this facility had some minor dings on its inspection report, it had zero severe deficiencies. Severe deficiencies are ones found in categories G, H, I, J, K and L. This tells you that CMS did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A few minor deficiencies aren't the end of the world.
Short-term Care Quality
We awarded this facility just a D for our short-term care rating and that actually turned out to be its next best category. In calculating our short-term care ratings, we quantify the nursing home's skilled nursing services, such as those performed by registered nurses, occupational therapists, physical therapists and other types of therapists. Our purpose is to devise a barometer for comparing the rehabilitation services of nursing homes. Based on its grade in this category, we were not stunned to find that this facility is well below average in terms of its number of physical therapy and registered nurse hours provided to its residents based on the measures we assessed. Honestly, this was what we expected to find. The final statistic we assessed in this area is the percentage of residents who who were able to eventually return home from the nursing home. This nursing home struggled quite a bit in this area as well, with just 0 percent of its residents returning home.

Long-term Care Quality
Additionally, we gave this facility an F in the category of long-term care. Nursing homes that receive this kind of score in long-term care likely don't provide the type of consistent around the clock care that some other facilities provide. On top of looking at the volume of care provided by nurses and other staff, we looked at the number of residents vaccinated against pneumonia. We were a bit bothered by the fact that this nursing home vaccinated only 58.436214 percent of its patients. To our surprise, this nursing home actually fared well at keeping its residents out of the hospital. In fact, it had just 0.05 hospitalizations per 1,000 long-term resident days. This is its best score in this category.
Nurse Quality
The next area we looked at is nursing, in which this nursing home received a rock bottom grade in this area as well. Unfortunately, we gave it a lowly F for this area, which is obviously a major concern. Nursing ratings are mostly associated with the nursing home's nurse staffing. This facility provided just 0.2 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. This place also had abysmal scores in some of the quality-based measures to go along with its weak nursing hours. We looked at the percentage of patients suffering pressure ulcers and we were very disappointed. This place was at approximately 1.5 times the national average in this measure. This is likely a bad sign when you consider that so many pressure ulcers could be avoided with better nursing care. This statistic helped pull down this nursing home's nursing grade significantly.
