Facility Inspections
The main reason this facility received a decent overall grade is its impeccable government inspections in recent years. We gave it one of our higher grades in that category, with an A. This is a much better grade than the place's overall grade, which was decent but certainly not elite. We take several aspects of a facility's inspection report into consideration in computing these inspection grades. One key factor is health deficiencies. You should keep in mind that the severity of these deficiencies is arguably more meaningful than the quantity of deficiencies, as some can be quite insignificant. While this facility had some deficiencies on its report, none of them were serious based on CMS' deficiency scale. A few minor deficiencies shouldn't lead you to cross a nursing home off your list.
Long-term Care Quality
The next best area we awarded this facility in any category came in the area of long-term care, where we gave it a C. For residents looking for a permanent place to live as opposed to skilled nursing, long-term care grades are an important measure. After considering the amount of care provided by aids and other staff, we next considered the facility's vaccination record. This nursing home vaccinated 99.34066 percent of its residents for pneumonia, which is higher than the vast majority of nursing homes. Lastly, this nursing home was also able to limit hospitalizations. It had only 0.01 hospitalizations per 1,000 long-term resident days, which is an impressively low figure.

Short-term Care Quality
Sadly, this facility only earned a D for its short-term care score, which is not a very good score. With our short-term care score, we try to create a valuable measure for rehabilitation services. In doing so, we analyze the nursing home's level of highly-skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. With a grade of this caliber in this category, we were not stunned to find that this place was below average in two of the key staffing figures we assess. In fact, it provided substantially fewer registered nurse and physical therapist hours per patient than most facilities. Finally, we looked at the percentage of residents who were able to return to the community from this nursing home. We found that just 5.2 percent of this facility's residents returned home.
Nurse Quality
Moving on to our final area, we awarded this nursing home a D in nursing. This was the facility's weakest category grade. In computing a nursing home's nursing grade, we consider the amount of time nurses are seeing residents as well as the levels of training of those nurses. This facility averaged just 0.4 hours of nursing care per resident per day. This statistic is far below average. Finally, we also looked at a few quality measures in determining our nursing ratings. This facility was above average in two of the areas we focus on, with good marks for minimizing its residents' major falls and pressure ulcers.
