Facility Inspections

Grade: A

To pair with its strong overall score, we awarded this place an A for the area of inspections. We take several aspects of a facility's inspection report into consideration in computing our inspection grades. One of those factors is health deficiencies. You should keep in mind that the severity of the deficiencies is arguably more important than the quantity of deficiencies, as some deficiencies are quite minor. Although this nursing home had a few deficiencies on its government inspection report, none were severe based on CMS' deficiency scale. The fact that none of the deficiencies were severe made us feel better about this inspection report.

Long-term Care Quality

Grade: A-minus

One of the other reasons we graded this nursing home so favorably is that it received a favorable long-term care score. In fact, long-term care turned out to be its second most favorable category score. In that area, we awarded this facility a score of A-. Facilities that receive this type of grade in long-term care tend to provide consistent around the clock care to ensure patients are well cared for. One of the statistics we considered in addition to nursing hours was vaccines. This nursing home administered the pneumonia vaccination to 67.88733 percent of its residents. Clearly, this place is doing something right in this area as it was able to keep its residents out of the hospital. In fact, it had only 0.01 hospitalizations per one thousand long-term resident days, which is a remarkably low figure.

Nurse Quality

Grade: C

Turning to the category of nursing care, this facility didn't perform quite as well in this area as some of the categories discussed above. Nevertheless, a grade of C in this category is by no means a bad score. Our nursing rating is mostly tied to the facility's level of nurse staffing. This facility provides 0.4 hours of nursing care per resident per day. This is a much lower figure than we were hoping for. Lastly, while this facility got dinged a bit for nursing hours per resident, it actually excelled in a few of our quality-based metrics. With less than five percent of its residents experiencing pressure ulcers, it performed very well in this metric.

Short-term Care Quality

Grade: D

Turning to the final area, we gave this facility a grade of D for the area of short-term care. This is the facility's worst category score. Unfortunately, this category pulled down the facility's overall grade to some degree. Our short-term care grade is generally employed to assess a nursing home's rehabilitation services. In order to have quality rehabilitation services, facilities generally must provide better levels of highly skilled nursing. Skilled nursing includes registered nurses, physical therapists, speech therapists and other types of therapists. In this nursing home's case, we found that it offered less registered nurse and physical therapist hours per resident than a typical facility. Finally, we looked at the percentage of residents who were able to return to the community from this nursing home. This was not a strength for this facility. We found that just 0 percent of this nursing home's patients returned home. At most nursing homes, around half of their residents are able to return home so this is well below average.

The Wanaque Center for Nursing & Rehabilitation Quality Metrics

Minimizes Pressure Ulcers

Grade: A-minus

In The Wanaque Center for Nursing & Rehabilitation, 5.62% of Patients had Pressure Ulcers

This figure tells you the percentage of long-term care patients who suffer from new or worsened pressure ulcers. We have found that pressure ulcers are a great indicator of a facility's nursing care quality.

Minimizes Serious Falls

Grade: A-plus

In The Wanaque Center for Nursing & Rehabilitation, 0.28% of Patients had Serious Falls

This datapoint is a measure of the percentage of long-term stay residents who have sustained falls which caused major injury. Falls can arise for a variety of reasons, but excessive numbers of falls could be an indicator of lower levels of patient supervision.

Minimizes Urinary Tract Infections

Grade: B-plus

In The Wanaque Center for Nursing & Rehabilitation, 0.99% of Patients had UTIs

This statistic gauges the percentage of long-term patients that have sustained UTI's. While a higher rate infections could reflect poorly on a nursing home's cleanliness, it is problematic to compare between facilities due to nursing homes having reporting standards.

Appropriately Uses Anti-Psychotic Medication

Grade: A

In The Wanaque Center for Nursing & Rehabilitation, 4.98% of Patients use Anti-Psychotic Medication

This indicates the percentage of residents prescribed antipsychotic drugs. While antipsychotic medications may be helpful for many residents, it is important to ensure these drugs are being used appropriately. In limited cases, high levels of antipsychotic drug use may suggest a facility is using these medications to control patient behavior.

Appropriately Uses Anti-Anxiety Medication

Grade: F

In The Wanaque Center for Nursing & Rehabilitation, 38.81% of Patients use Anti-Anxiety Medication

This metric is an indication of the percentage of long-term patients receiving antianxiety drugs.

Managing Depression Among Residents

Grade: C

In The Wanaque Center for Nursing & Rehabilitation, 9.73% of Patients

This is the percentage of patients showing symptoms of depression. Higher levels of depression could indicate worse patient care.

Appropriate Vaccine Usage

Grade: D

In The Wanaque Center for Nursing & Rehabilitation, 81.72% of Patients

This metric measures the percent of long-term stay patients who have received the pneumonia and flu vaccines. Respiratory viruses can be fatal for nursing home patients, making these vaccines indispensable.

Residents Maintain Autonomy

Grade: A-plus

In The Wanaque Center for Nursing & Rehabilitation, 4.49% of Patients

This is the percent of patients who needed increased assistance with activities of daily living over time. High levels of dependence on staff for assistance with ADL's may be a sign of the decline of a patient's well-being.

Ability to Keep Residents Mobile

Grade: D

In The Wanaque Center for Nursing & Rehabilitation, 8.81% of Residents

This datapoint measures the percent of long-term stay residents who were able to retain mobility. Some experts would argue that the ability to move around is critical to residents' physical and mental well-being.

In The Wanaque Center for Nursing & Rehabilitation, 0.01 Hospitalizations per 1,000 resident days

This is the number of hospitalizations per thousand days of patient care.

Short-term Care: Rehospitalizations

Grade: A

In The Wanaque Center for Nursing & Rehabilitation, 8.81% of Residents Rehospitalized

This is the number of rehospitalizations per thousand days of short-term patient care.

Short-term Care: Facilitates Functional Improvement

Grade: C

In The Wanaque Center for Nursing & Rehabilitation, 72.02% of Resident

This metric is a measure of the percentage of short-term care patients that saw functional improvements, such as with activities of daily living. Some argue this is a measure of rehabilitation performance.
