This was right in line with the city grade in Lady Lake. You could certainly do much worse than this nursing home. The best part of this facility's strong profile was its inspection rating. Facility Inspections

Grade: A-minus

This nursing home received a quality overall score as described above, but we especially wanted to emphasize its excellent inspections in recent years. We gave it an A- in this category. These inspection ratings weigh several factors, including deficiencies and substantiated complaints. You can learn more about each of these factors by obtaining copies of nursing homes' inspection reports. This facility was hit with 3 deficiencies on its inspection report, but none of the deficiencies were considered to be severe deficiencies. This tells you that CMS didn't consider any of these deficiencies to create an immediate risk to patient safety or health. We should point out that deficiency-free inspection reports are rare in this industry.

Short-term Care Quality

Grade: A-minus

Adding to this facility's list of strengths, it also received a top-shelf short-term care score. We awarded them an elite grade of A- in this area. Short-term care ratings are based in part on a facility's quantity of skilled nursing services. This includes a broad range of nursing services, ranging from registered nurses to physical and occupational therapists, as well as other forms of therapy. Fortunately, it appear that this facility has registered nurses on staff. Not every nursing home employs these skilled professionals. On the other hand, based on the data this nursing home provided, they do not appear to employ physical therapists. The final metric we considered in this area is the number of residents who were able to leave the facility and return to the community. We found that 5.8 percent of this facility's residents returned home.

Long-term Care Quality

Grade: B-minus

We also gave this facility a middle of the pack grade in the category of long-term care, where we awarded it a grade of B-. For residents in need of a permanent place to live as opposed to rehabilitation, long-term care grades are a key measure. On top of assessing the amount of care provided by nurses aids and other staff, we also looked at the number of residents vaccinated against pneumonia. This nursing home administered the vaccine to 100 percent of its patients, which is higher than the average nursing home. Finally, this nursing home was also able to limit hospitalizations. In fact, it had only 0 hospitalizations per 1,000 long-term resident days, which is a remarkably low figure.

Nurse Quality

The last category we looked at was nursing, proved to be this nursing home's weakest category. We awarded this facility a grade of D in this category. This really was one of just a few weak links in an otherwise impressive report card. Nursing grades are mostly based on the nursing home's nurse staffing.
