Facility Inspections
The main reason this facility received a decent overall score is its impeccable health inspections in recent years. We gave it one of our higher grades in that area, with an A-. This is a notably better grade than the facility's overall score, which was decent but not in the elite range. Inspection grades weigh several factors, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these issues by reviewing copies of nursing homes' inspection reports. This nursing home was hit with 4 deficiencies on its inspection report, but none were considered to be severe deficiencies. This tells you that CMS didn't consider any of these deficiencies to create an imminent threat to resident health or safety. We should point out that deficiency-free inspections are rare in this industry.
Short-term Care Quality
We also would like to note that this nursing home was awarded a strong short-term care grade. Indeed, we gave it one of our most favorable scores in that category with a score of A-. In determining our short-term care ratings, we assess the nursing home's levels of highly skilled nursing, which includes registered nurses, physical therapists respiratory therapists and other types of therapists. This grade is more often than not a meaningful measure of a nursing home's rehabilitation. This nursing home boosted its score in this category by offering more physical therapy hours to its residents than the average facility. The last metric we looked at in this category is the number of residents who were able to leave the nursing home and return to the community. We found that it performed as well as just about any nursing home in Ohio in this area with 71.2 percent of its residents returning home. At most facilities, fewer than half of their short-term residents are able to return home.

Nurse Quality
Another strength of this facility is its favorable grade in nursing. We awarded them one of our better scores in that area, with an D. We weighed the licensure of nurses at the nursing home, as well as the number of hours those nurses were with residents, in determining our grade in this category. In this nursing home's case, quantity of care does not appear to be the problem. According to its CMS data, this facility provided an average of 4.1 hours of nursing care per resident daily, which was above the national average. Despite performing well in terms of nursing hours, this place did not fare as well in the quality-based metrics we looked at in computing our nursing scores. We looked at the percentage of residents sustaining major falls and pressure ulcers. This facility was above the national average in both statistics. This is not a good sign when you consider that many falls and bed sores are preventable with better nursing care. Unfortunately, these metrics dragged down this facility's nursing score quite a bit.
Long-term Care Quality
Turning to our final area, this facility really did not fare very well. Due to a very poor grade of F in long-term care, this was this nursing home's weakest area. Long-term care scores of this caliber generally are a bad sign for the quantity and quality of care provided by nurses and aids. Even though we determined this nursing home had an above average amount of nursing care provided by nurses and other nursing staff, we found its vaccine statistics to be a bit slightly lower than we anticipated. It administered the pneumonia vaccine to just 89.928055 percent of its residents. This is one statistic we would really like to see this facility improve. To our surprise, this facility was actually able to limit hospitalizations. With only 0 hospitalizations per one thousand long-term resident days, this nursing home has fewer hospitalizations than many nursing homes. This was its best feature in this category.
