Arguably the most critical factor we consider in determining our inspection grades is deficiencies. Deficiencies can be found on a facility's recent inspection reports. Places with higher grades in this area typically dodged the most severe deficiencies involving health risks or even death. This particular nursing home had just a single deficiency on its inspection report and it was not one deemed to pose a threat to patient health or safety. This is one of the most highly highly trained levels of nurses. On top of offering high levels of nursing care, this facility also excelled in the quality-based metrics we assessed. For example, it performed as well as any facility in the state in the area of minimizing its residents' major falls and pressure ulcers. We view these areas as reliable indicators of the caliber of nursing care offered.

This facility also was awarded an impressive short-term care grade. In fact, we gave it an above average grade of B+ in this category. In our short-term care score, we attempt to craft a fair gauge for rehabilitation services. In this process, we look at a nursing home's scope of highly-skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. This place provided more physical therapist and registered nurse hours per resident than most nursing homes. This is what we like to find when assessing a nursing home in this category. The final metric we assessed in this category is the number of residents that ultimately returned home from the facility. We found that 0 percent of this nursing home's patients returned home as opposed to remaining at the nursing home on a permanent basis.

Unfortunately, we were not able to track down this facility's vaccination rates. Clearly, this facility is doing something right in this area as it also excelled at keeping its residents out of the hospital.
