Facility Inspections
While we were not very complementary of this nursing home's overall grade, it actually received a very impressive inspection report this year. In fact, it received a nearly flawless inspection report this year. As a result, it earned one of our highest grades in that category with a grade of A-. Perhaps the most significant factor we look at in calculating our inspection ratings is deficiencies. Deficiencies are found on a nursing home's inspection reports. It is typically in your best interest to avoid nursing homes that have too many deficiencies. This place was hit with 5 deficiencies on its inspection report, but we were relieved to see that none of the deficiencies were considered to be severe deficiencies. This means that the inspectors did not consider any of these deficiencies to create an immediate risk to resident safety or health. A couple minor deficiencies aren't necessarily the end of the world.
Nurse Quality
One of the other reasons we rated this facility so highly is that it earned a very impressive nursing score. This turned out to be its second best category grade. In that category, we awarded this nursing home an D. Our nursing rating assesses a handful of subcategories, however, the main one is the number of nurse hours spent with patients. This facility averaged only 0.3 hours of nursing care per patient daily. Finally, we also looked at some nursing quality-based metrics in computing our nursing scores. This facility performed well in terms of minimizing its residents' pressure ulcers and major falls.

Long-term Care Quality
This facility's second lowest score was long-term care, where it received a grade of just D in that category. This is a relatively poor grade. When nursing homes receive a grade in this range in this category it generally means the facility didn't perform well in our quality measures relating to resident care. After considering the quantity of care provided by aids and other staff, we next considered the facility's vaccination records. This facility vaccinated 92.83276 percent of its patients against pneumonia, which is a bit lower than we expected but still a reasonably acceptable figure. Surprisingly, this facility was actually decent at keeping its patients out of the hospital. In fact, it had just 0.02 hospitalizations per one thousand long-term resident days. This is its best score in this category.
Short-term Care Quality
The next category we scored was short-term care, ended up being this nursing home's least impressive area. We awarded this facility a D in this area. Our short-term care scores are meaningful for residents requiring rehabilitation. Rehabilitation usually mandates more highly-skilled nursing services. This means a wide spectrum of services, spanning from registered nurses to physical and respiratory therapists, as well as other types of therapy. With its score in this category, we were not stunned to learn that this facility is well below average in terms of its number of registered nurse and physical therapy hours provided to its patients based on the data we assessed. Finally, we considered the percentage of patients who were able to return to the community from this facility. We found that just 0 percent of this nursing home's patients returned home.
