The Meadows is an average-sized non-profit nursing home located in Nashville, Tennessee. This facility is one of the most elite nursing homes we assessed. A grade of this caliber requires superb scores across the board. We were not surprised to discover that this is one of the three best facilities in the city. Facility Inspections

Grade: A-plus

We also wanted to point out that this nursing home earned near flawless government inspections recently. We gave them an impressive grade of A+ in this category. Inspection grades take several factors into consideration, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these items by reviewing copies of nursing homes' inspection reports. This place was assessed 5 deficiencies on its inspection report, but fortunately none of the deficiencies were considered to be severe. This indicates that the government inspectors didn't consider any of the deficiencies to cause an imminent risk to patient safety or health. Remember that deficiency-free inspection reports are rare in the industry.

Nurse Quality

Grade: A-plus

One of the other reasons we graded this facility so highly is that it earned a very impressive nursing grade. This turned out to be its second best category grade. In that category, we gave this facility an A+. Our nursing rating is based on many factors. The most heavily weighted one is the quantity of hours nurses spent with patients. This place offers an impressive 4.4 hours of nursing care per patient per day. This is among the highest totals in Tennessee. Finally, this place was also above average in each of the major quality-based metrics we assessed in this category. For example, it performed well when it comes to minimizing its residents' major falls and pressure ulcers. These areas are generally reliable indicators of the quality of nursing care a nursing home provides.

Short-term Care Quality

Grade: A-minus

This facility also was superb in the category of short-term care, where we gave it a score of A-. Very few facilities performed better in this area. In the category of short-term care, we try to qualify measures of a facility's rehabilitation services. We analyze the facility's skilled nursing services, such as the ones performed by registered nurses and various therapists. One of this nursing home's strength is that it offers more hours of care from registered nurses to its residents. The final datapoint we assessed in this category is the percentage of patients that were able to return home from the nursing home. We found that it fared better than most facilities in this area with 52.7 percent of its patients returning home. At most facilities, fewer than half of their short-term residents are able to return home.

Long-term Care Quality

Grade: B

The next category we looked at proved to be this facility's poorest category. Nevertheless, even its weakest link would be a strength for most nursing homes. We awarded it an above average grade of B for long-term care. When facilities receive a grade in this range in this category it generally means it's well-staffed and is an overall good place to reside on a permanent basis. On top of assessing the quantity of care provided by aids and other staff, we also looked at the number of residents vaccinated for pneumonia. This facility vaccinated 95.588234 percent of its residents, which is more than the vast majority of nursing homes. Lastly, this facility was able to limit hospitalizations. With only 1.66 hospitalizations per one thousand long-term resident days, this nursing home had less hospitalizations than the average nursing home.
