While this facility's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a grade of C for that category, which is one of our better scores. We believe that one of the most critical factors in evaluating an inspection report is deficiencies. In this regard, we especially focus on the severity of the deficiencies. This facility had 3 deficiencies on its inspection report, but none were in any of the categories labeled G through L. This indicates that inspectors didn't consider any of the deficiencies to create an imminent risk to patient health or safety. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.

This facility's next most favorable score is short-term care, but it nevertheless received a grade of just D in that category. This is still a relatively poor grade. In computing our short-term care ratings, we look at a facility's levels of skilled nursing, which includes registered nurses, physical therapists occupational therapists and other highly trained professionals. This grade is typically a useful measure of a nursing home's rehabilitation. Not surprisingly, we found that this place offers far less registered nurse and physical therapist hours per patient than the typical facility. Finally, we looked at the number of residents who were able to return to the community from this nursing home. We found that just 5.5 percent of this facility's patients returned home.

Unfortunately, we gave this facility an F for its nursing grade. In determining our nursing ratings, we factor in both staffing levels and the training levels of those nurses. This particular place offered a meager 0.3 hours of nursing care per resident daily. This number is far below average. Lastly, we also looked at some nursing quality-based metrics in determining our nursing scores. This nursing home performed well in the area of avoiding pressure ulcers and major falls. We consider these metrics to be good measures of the quality of nursing care.

The next category we graded was long-term care. It received an F in this category, which is an abysmal score. When nursing homes receive a score in this range in long-term care it typically means the facility didn't perform well in our quality measures relating to patient care. On top of considering the volume of care provided by nurses and other staff, we analyzed the percentage of residents vaccinated for pneumonia. Candidly, we were a bit alarmed this facility administered the vaccine to just 73.33333 percent of its patients. Surprisingly, this place actually fared well at keeping its residents out of the hospital. In fact, it had just 0 hospitalizations per 1,000 long-term resident days. This is its best feature in this category.
