The Enclave is a large nursing home located in San Antonio, Texas. We gave this facility a rock bottom overall rating based on the data we looked at. San Antonio received a city grade of C, so there are other options in the city worth looking at. Despite all this, one of the few highlights of this facility's profile is its strong inspection reports. You can scroll down to find out about inspections and other category grades

Facility Inspections
While we gave this place a terrible overall grade, we awarded it a B+ for our inspections category. This is due to the nursing home receiving a quality inspection report this year. We weigh several aspects of a nursing home's inspection report in determining these inspection ratings. One critical factor is health deficiencies. You should keep in mind that the severity of deficiencies is usually more important than the number of deficiencies, as some of these are quite minor. While this nursing home had a few minor dings on its inspection report, it had zero severe deficiencies. Severe deficiencies are ones labeled as categories G through L. This tells you that the inspectors did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A few minor deficiencies are not necessarily the end of the world.
Short-term Care Quality
Surprisingly, we gave this facility a C for its short-term care rating, which isn't too bad of a score. Our short-term care grade is often used to score a nursing home's performance with rehabilitation. In order to provide highly graded rehabilitation services, facilities generally need to have higher levels of skilled nursing services. Skilled nursing includes registered nurses, physical therapists, speech therapists and other highly trained professionals. One of this nursing home's strengths is physical therapy hours. We found that it offers more hours with physical therapists to its residents than most facilities. Finally, we considered the number of patients who were able to return to the community from this nursing home. It outperformed the majority of nursing homes in the country with 54.4 percent of its patients returning home.
Long-term Care Quality
Another issue was the F this facility was given in the area of long-term care. When nursing homes receive a grade in this range in long-term care it typically means the facility did not perform well in our quality measures relating to patient care. Once we assessed the quantity of care provided by nurses, we next considered the nursing home's vaccination records. Frankly, we were a bit concerned this nursing home vaccinated just 54.4484 percent of its patients against pneumonia. Finally, we were stunned by its number of hospitalizations. Here we found that this place fared poorly with 3.62 hospitalizations per one thousand long-term resident days. Sadly, this figure is more than double the national average.
Nurse Quality
The final category we analyzed is nursing. This nursing home was given an abysmal grade of F in that area. In computing a nursing home's nursing grade, we weigh the number of hours nurses are seeing patients and the level of skill of the nurses. This place provided just 3.2 hours of nursing care per resident each day. This is an alarmingly low total. This place also had abysmal scores in some of the quality-based metrics to pair with its low nursing hour totals. We looked at the percentage of residents sustaining pressure ulcers and we were quite disappointed. This facility was at approximately 150 percent of the national average in this area. This is likely a bad sign in light of the fact that many bed sores could be avoided with better nursing care. This metric helped pull down this nursing home's nursing grade significantly.
