Facility Inspections
We want to draw your attention to the fact that this facility really outperformed its overall score in the category of inspections. In fact, it received a nearly flawless inspection report this year. As a result, we gave it one of our best scores in this category with an A. Perhaps the most critical factor we look at in calculating our inspection scores is deficiencies. These deficiencies are found on a nursing home's inspection reports. Facilities with better grades in this area most likely avoided the more severe deficiencies involving patient abuse or death. Although this nursing home had a few deficiencies on its report, none were major deficiencies based on CMS' deficiency scale. A couple of minor deficiencies should not lead you to cross a nursing home off your list.
Short-term Care Quality
Another one of this facility's higher category grades came in the category of short-term care. In that area, we gave this nursing home a B. In calculating these short-term care grades, we analyze the nursing home's skilled nursing services, such as those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. Our purpose is to formulate a measure for comparing the rehabilitation services of different nursing homes. Unfortunately, it doesn't appear that this facility submitted data regarding its registered nurse or physical therapy staffing. Finally, we assessed the number of patients who were able to return to the community from this facility. We found that 6.5 percent of this facility's residents returned home.

Long-term Care Quality
Turning to the area of long-term care, this facility received a grade of just D in this category. In a long-term care environment, the nursing home's primary goal is to maintain residents' quality of life and keep them safe. In addition to considering the volume of care provided by nurses and other staff, we analyzed the percentage of residents vaccinated against pneumonia. This nursing home provided the vaccine to 94.37751 percent of its residents. Surprisingly, this facility was able to limit hospitalizations. With just 0.02 hospitalizations per one thousand long-term resident days, this facility had fewer hospitalizations than the average nursing home. Unfortunately, some of its other scores in the category weren't as favorable as these.
Nurse Quality
The final category we looked at was nursing. It received an F in this category, which is an abysmal grade. This is obviously a major disappointment. We looked at the levels of nurses working for the facility, in addition to the number of hours the nurses spent with residents, in computing our score in this area. This facility provided just 0.3 hours of nursing care per patient daily. This is well below average. To go along with its subpar totals in the area of nursing hours per resident, this nursing home also didn't fare as well in the quality-based measures we looked at in calculating our nursing scores. We looked at the percent of patients sustaining pressure ulcers and major falls. This place had more pressure ulcers and falls than the average facility.
