Facility Inspections

Grade: A

While we were fairly low on this facility overall, we awarded it an A for our inspections rating. This score is significantly more impressive than the nursing home's overall grade. Our inspection grades are tied to many items found on the a nursing home's recent inspection reports. Deficiencies are a key item to look for on these inspections. Most importantly, you should avoid facilities that have severe deficiencies linked to risks to resident well being. Fortunately, although this nursing home had some minor dings on its government inspection report, it had no severe deficiencies. Severe deficiencies are ones labeled as categories G, H, I, J, K and L. This means that CMS did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A few minor deficiencies aren't necessarily the end of the world.

Long-term Care Quality

Grade: C

This facility's second most favorable area ended up being long-term care. In that area, we awarded this nursing home a C. In calculating our long-term care ratings, we assess the assistance with daily living received by the nursing home's residents. Once we concluded our assessment of the quantity of care provided by nurses, we then looked at the facility's vaccination record. We were pleased to learn that this nursing home vaccinates 96.56161 percent of its patients against pneumonia. Finally, this place was also able to limit hospitalizations. It had only 0 hospitalizations per one thousand long-term resident days, which is an impressively low figure.

Short-term Care Quality

Grade: D

Sadly, this nursing home only received a D for its short-term care score, which is not a very good score. Our short-term care scores are critical for people looking for rehabilitation. Rehabilitation usually requires additional skilled nursing. Skilled nursing includes a wide range of nursing services, ranging from registered nurses to physical and occupational therapists, as well as other forms of therapy. With its score in this category, we weren't surprised to find that this place is well below average in terms of its number of registered nurse and physical therapy hours offered to its residents based on the metrics we looked at. Honestly, this was what we expected to find. Lastly, we looked at the number of residents who were able to return to the community from this nursing home. This definitely wasn't a strength for this nursing home. We found that just 4.7 percent of this nursing home's residents returned home. At most facilities, about half of their residents are able to return home so this is well below average.

Nurse Quality

The last category we looked at is nursing, in which this nursing home was given a poor grade in this category also. We gave it an abysmal F for this area, which is definitely somewhat alarming. Our nursing grade looks at the facility's level of nurse staffing. We consider both the levels of skill of the nurses as well as the number of hours spent with residents. To pair with its low total nursing hours per resident, this facility had poor scores in several of the quality-based measures we looked at in computing our nursing ratings. For starters, we looked at the percent of residents sustaining falls leading to major injury. This facility was at more than 1.5 times the national average in this statistic.
