The Brazos of Waco is a large nursing home located in Waco, Texas. With a really poor overall grade of F, this facility isn't high on our list. Although this facility's overall grade was as bad as it gets, it actually didn't perform terribly in the area of short-term care. We gave it a B- for that category, which is a decent score. However, this wasn't enough to spare its profile. Our short-term care grades are meaningful for people looking for rehabilitation. Rehabilitation generally requires higher levels of highly-skilled nursing. This includes a wide range of services, ranging from registered nurses to physical and occupational therapists, as well as other types of therapy. We found that this facility was below average in the two key staffing areas we assessed. It offered fewer physical therapist and registered nurse hours per resident than most other nursing homes. The final measure we looked at in this category is the number of residents who were able to return home from the nursing home. This place fared much better in this area with 60.4 percent of its residents returning home.
Nurse Quality
One of the other reasons we rated this facility so highly is that it received a very impressive nursing rating. Nursing turned out to be its second best category grade. In that area, we gave this facility an D. When determining our nursing scores, we look at both the number of hours nurses spend with residents and the levels of licensure of those nurses. This particular place averaged just 3.2 hours of nursing care per resident each day. This number is quite a bit below average. In addition to its weak nursing hours, this place also received poor scores in several of the quality-based metrics we considered in computing our nursing ratings. We found that a higher percentage of this facility's patients sustained falls leading to serious injury.
Facility Inspections
Unfortunately, with a grade of D in the inspection category, this facility performed relatively poorly in that area. One of the most critical factors in assessing an inspection report is deficiencies. We would scrutinize the severity of the deficiencies. This facility was assessed 2 deficiencies on its inspection report. The only positive thing we found is that none of the deficiencies were in the categories that suggest they posed a threat to resident safety or health. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Long-term Care Quality
The last category we graded was long-term care, which is this facility's least impressive area. We awarded this nursing home a D in this area. For residents seeking a permanent residence rather than rehabilitation, long-term care is a very important category. Once we assessed the volume of care provided by aids and other staff, we turned to the facility's vaccination record. We were optimistic to learn that this nursing home vaccinated 100 percent of its residents for pneumonia. Finally, we looked at the nursing home's number of hospitalizations. With 1.9 hospitalizations per one thousand long-term resident days, this place didn't fare quite as well.
