Facility Inspections
While we gave this nursing home a poor overall grade, we awarded it a grade of B+ for our inspections category. This is due to the facility receiving a decent inspection report this year. Our inspection scores weigh a host of factors found on a facility's inspection report. One of the most important criteria we look at is the quantity and severity of deficiencies. You generally want to avoid places with a list of severe deficiencies flagged. While this facility had some deficiencies on its inspection report, none were serious based on CMS' scale. A few minor deficiencies are not the end of the world.
Long-term Care Quality
Sadly, this facility only earned a grade of D for its long-term care rating, which ended up being one of its better category scores. Nursing homes that receive this type of score in this category likely don't provide the type of consistent around the clock care that some other facilities provide. In addition to considering the quantity of care provided by nurses aids and other staff, we also looked at the percentage of residents vaccinated for pneumonia. As it turns out, this nursing home vaccinated just 22.807016 percent of its patients, which is quite a bit lower than we expected. Surprisingly, this place was actually decent at keeping its residents out of the hospital. In fact, it had only 0.01 hospitalizations per one thousand long-term resident days. This is its best feature in this category.

Nurse Quality
This facility ended up receiving rock bottom score in our nursing category. It received an F in this category. Our nursing rating considers many factors. The most heavily weighted variable is the quantity of hours nurses spent with residents. This nursing home averaged only 0.3 hours of nursing care per resident each day. This is an alarmingly low total. Lastly, we also assessed some nursing quality measures in computing our nursing ratings. This place fared well in the area of avoiding major falls and pressure ulcers. We consider these areas to be indicators of the quality of nursing care.
Short-term Care Quality
Moving on to our final area, this facility didn't fare well in this category either. Due to an abysmal grade of F in short-term care, this is just about as terrible as it gets. With our short-term care assessment, we strive to craft a fair gauge for rehabilitation services. In doing so, we analyze a nursing home's levels of highly-skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Finally, we looked at the percentage of residents who were able to return home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 0 percent of this facility's patients were able to return home. This figure was well off the national average.
