Facility Inspections
While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. Our inspection ratings account for a host of factors found on a nursing home's inspection report. One key criteria we rely on is the quantity and severity of deficiencies. While this facility had a few deficiencies on its report, none were serious based on CMS' scale. A few minor deficiencies aren't necessarily the end of the world.
Nurse Quality
Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. There are numerous factors within this category. Most of the criteria are tied to levels of nurse staffing. This nursing home provides 0.3 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Finally, we also assessed some nursing quality measures in calculating our nursing scores. This place performed well in the area of avoiding major falls and pressure ulcers.

Short-term Care Quality
This facility wound up receiving a rock-bottom grade in our short-term care area. It received a lowly F in this area. In the area of short-term care, we try to evaluate indicators of a facility's rehabilitation. We look at a nursing home's skilled nursing services, including the ones performed by registered nurses and physical and occupational therapists. Considering its score in this area, we were not surprised to find that this nursing home is well below average in terms of its number of physical therapy and registered nurse hours provided to its patients based on the data we looked at. Lastly, we considered the percentage of patients who were able to return to the community from this nursing home. This was not a strength for this nursing home. We found that just 3.9 percent of this facility's patients returned home. At most nursing homes, about half of their patients are able to return home so this is well below average.
Long-term Care Quality
Turning to our next area, this nursing home really didn't perform well in this category either. With a bottom of the barrel grade of F in long-term care, this is just about as terrible as it gets. Facilities that receive this type of score in this category may not provide the type of consistent 24/7 care that some other nursing homes offer. One of the factors we considered after nursing hours was vaccines. Frankly, we were a bit bothered by the fact that this facility provided the pneumonia vaccine to only 37.386017 percent of its patients. Surprisingly, this nursing home actually fared well at keeping its residents out of the hospital. It had just 0 hospitalizations per 1,000 long-term resident days. This is its best feature in this category.
