Facility Inspections
While this facility's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a B- for that category, which is one of our better scores. Inspection grades are tied to many datapoints found on the a nursing home's recent inspection reports. Deficiencies are a key item to look for on these reports. Severe deficiencies indicate a threat to the well being of residents. While this place had a few deficiencies on its report, none were severe based on CMS' deficiency scale. Lastly, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
Long-term Care Quality
We were also surprised to learn that this nursing home received a decent long-term care score in our assessment. We awarded it a grade of B- in that category. If you are seeking anything other than rehabilitation, you should take a close look at each nursing home's long-term care scores. On top of assessing the quantity of care provided by aids and other staff, we analyzed the number of residents vaccinated for pneumonia. Fortunately, this nursing home provided the vaccine to 100 percent of its patients, which is a very solid figure. Finally, this facility was also able to limit hospitalizations. It had only 0.02 hospitalizations per one thousand long-term resident days, which is a remarkably low number.

This is an alarmingly low total. Finally, we also assessed some nursing quality-based metrics in computing our nursing ratings. Short-term Care Quality
The last category we scored was short-term care. This nursing home received a rock bottom grade of F in this category. Our short-term care ratings are thought to be more important for those needing rehabilitation services. Rehabilitation typically requires additional skilled nursing. Skilled nursing includes not merely nursing, but also physical and respiratory therapy, as well as other types of therapy. With its grade in this category, we were not stunned to learn that this facility is well below average in terms of its number of registered nurse and physical therapy hours offered to its patients based on the measures we assessed. Honestly, this was what we expected here. Finally, we assessed the number of patients who were able to return to the community from this nursing home. We found that just 3 percent of this facility's residents returned home.
