Facility Inspections
While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. We weigh several aspects of a nursing home's inspection report in calculating our inspection ratings. One key factor is health deficiencies. You should keep in mind that the severity of the deficiencies is usually more important than the number of deficiencies, as some are relatively minor. This facility was hit with 5 deficiencies on its inspection report, but we were relieved to see that none were considered severe. This tells you that CMS did not deem any of the deficiencies to be an imminent risk to patient safety or health. A couple minor deficiencies are not the end of the world.
Nurse Quality
Turning to the area of nursing care, this facility received a grade of just D in this category. Unfortunately, this ended up being one of its better grades. Our nursing score is largely associated with a facility's level of nurse staffing. This nursing home provided a meager 0.3 hours of nursing care per patient on a daily basis. This number is quite a bit below average. Finally, we also looked at some nursing quality measures in determining our nursing scores. This facility fared well when it comes to avoiding pressure ulcers and major falls.

Long-term Care Quality
The third category we assessed was long-term care. This facility's was given a bottom of the barrel long-term care score where it received a lowly F. Facilities that don't fare well in long-term care typically aren't as well-staffed and are lagging in a few of the areas of routine personal care we looked at. One of the factors we considered after nursing hours is vaccines. Frankly, we were a bit worried that this facility administered the pneumonia vaccination to only 88.039215 percent of its patients. Surprisingly, this facility was actually decent at keeping its residents out of the hospital. In fact, it had only 0.02 hospitalizations per one thousand long-term resident days. This is its best feature in this category.
Short-term Care Quality
The next area we assessed is short-term care. This nursing home was given an abysmal grade of F in that category. In forming our short-term care scores, we look at the nursing home's skilled nursing services, including those performed by registered nurses, respiratory therapists, physical therapists and other types of therapists. Our objective is to create a scale for comparing the rehabilitation services of different nursing homes. Given its grade in this category, we weren't stunned to learn that this nursing home was well below average in terms of its number of registered nurse and physical therapy hours offered to its patients based on the data we assessed. Honestly, this was about what we expected here. Lastly, we looked at the percentage of residents who ultimately returned home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 3.3 percent of this facility's patients were able to return home. This figure was well off the national average.
