Facility Inspections
Although this nursing home's overall score was as bad as it gets, it actually fared pretty well in the area of inspections. In fact, we awarded it a B+ for that area, which is one of our better scores. Our inspection ratings are based on pieces of information located in the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these inspections. You should especially avoid facilities with severe deficiencies associated with endangerment of patients. Although this place had some deficiencies on its government inspection report, none were major deficiencies based on CMS' scale. A few minor deficiencies are not necessarily the end of the world.
Long-term Care Quality
This facility also didn't fare terribly in the area of long-term care. In fact, we gave it a C in that category, which happens to be among its more respectable scores. Long-term care grades are often used to judge a nursing home's performance as a traditional convalescent home, as opposed to focusing more on the skilled nursing services provided by a rehabilitation facility. On top of assessing the amount of care provided by aids and other staff, we also looked at the percentage of residents vaccinated for pneumonia. This facility provided the vaccine to 98.51852 percent of its residents, which is more than the majority of nursing homes. This facility was able to keep its patients out of the hospital. In fact, it had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low figure.

Short-term Care Quality
This facility ended up receiving a a terrible score in our short-term care area. It was given an F in this category. In forming these short-term care ratings, we quantify the facility's skilled nursing services, such as those performed by registered nurses, respiratory therapists, physical therapists and other types of skilled professionals. Our purpose is to create a tool for sizing up the rehabilitation services of various nursing homes. Considering its grade in this category, we were not stunned to learn that this place is well below average in terms of its number of registered nurse and physical therapy hours provided to its residents based on the metrics we assessed. Honestly, this was what we expected here. The last measure we looked at in this category is the number of residents that were able to return home from the nursing home. This nursing home struggled quite a bit in this metric as well, with just 4.6 percent of its residents returning home. Unfortunately, this was quite a bit below average.
Nurse Quality
The final area we scored was nursing. This nursing home received an abysmal grade of F in this area. The nursing grade weighs several components, most of which are associated with nursing hours. This particular nursing home offered a meager 0.3 hours of nursing care per patient each day. This figure is quite a bit below average. In addition to ranking below average in nursing hours per resident, this nursing home also had poor marks in the quality-based measures we looked at in computing our nursing scores. We looked at the percent of patients experiencing major falls and pressure ulcers. This place was at approximately 150 percent of the national average in both of these metrics. This is likely a bad sign when you consider that many falls and bed sores are believed to be preventable with better nursing care. These scores pulled down this facility's nursing rating significantly.
