While this facility's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a B- for that category, which is one of our better scores. We believe that one of the most critical factors in assessing an inspection report is deficiencies. In this regard, we especially look at the severity of the deficiencies. This facility received 4 deficiencies on its inspection report, but none were considered to be major deficiencies. Incredibly, we gave this nursing home a grade of B- for our short-term care score, which is not a bad grade. In calculating our short-term care ratings, we analyze the nursing home's levels of skilled nursing services, which includes registered nurses, physical therapists speech therapists and other licensed professionals. This score is generally a solid measure of a facility's ability to rehabilitate patients. One of this nursing home's strengths is physical therapy hours. We found that it offers more hours with physical therapists to its residents than most facilities. The last measure we assessed in this area is the number of patients who were able to leave the facility and return to the community. We found that it was above average in this metric with 53.3 percent of its residents returning home.

Turning to the final area, we awarded this facility a D in our long-term care category. This was the facility's least impressive category grade. In a long-term care environment, the nursing home's primary objective is to maintain patients' quality of life and keep them safe. After considering the quantity of care provided by nurses and other staff, we turned to the facility's vaccination record. We were optimistic to find that this facility vaccinated 100 percent of its patients for pneumonia. Unfortunately, its hospitalization rate was far less impressive. Here we found that this facility had 2.2 hospitalizations per 1,000 long-term resident days, which is an alarming rate.
