To go along with its favorable overall grade, we awarded this facility a grade of A+ for our inspections rating. Our inspection scores weigh a host of factors included in a facility's inspection report. One key criteria we weigh heavily is the number and severity of deficiencies. Facilities with higher grades in this category generally have few of these severe deficiencies. Although this place had a few deficiencies on its inspection report, none were serious based on CMS' deficiency scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Another reason we assessed this facility so highly is that it was given an A+ long-term care score. This is flat out as strong as it gets in this area. Facilities that receive this kind of score in long-term care typically provide consistent around the clock care to make sure patients are well cared for. In addition to looking at the impressive volume of care provided by nurses aids and other staff at this facility, we also were pleased by the facility's record for vaccinations. By way of example, this nursing home administered the pneumonia vaccine to 100 percent of its residents, which is what we like to see. This combination proved to be effective as this place was able to keep its patients out of the hospital. In fact, it had only 1.29 hospitalizations per 1,000 long-term resident days, which is a much better hospitalization rate than the majority of nursing homes.

Adding to its impressive resume, this nursing home also excelled in our nursing category. In fact, we awarded it a grade of A for that category. Our nursing grade analyzes the facility's level of nurse staffing. We consider both the levels of training of the nurses and the number of hours spent with residents. This particular nursing home provided 4.1 hours of nursing care per resident on a daily basis, which was among the better totals we found. Lastly, this place also excelled in the quality-based metrics we looked at. For example, it performed as well as any facility the nation when it comes to minimizing its residents' major falls and pressure ulcers. We look at these areas as predictive indicators of the quality of nursing care being provided.

Switching gears to the final area, this facility received a decent short-term care score. Although this was not quite as elite as several of its other grades, this is really not a poor grade. In our short-term care grade, we seek to craft a meaningful gauge for rehabilitation services. In doing so, we analyze a nursing home's levels of highly-skilled nursing services, including both registered nurses and physical therapy, as well as speech therapy. One of this nursing home's strengths is physical therapy hours. We found that it offers more hours with physical therapists to its residents than most facilities. Finally, we looked at the percentage of patients who returned home from this facility. We found that 31 percent of this facility's patients were able to return home. This figure was actually below the national average.
