Facility Inspections
While we did not rate this nursing home well overall, we want to note its decent health inspection reports in recent years. We awarded it a somewhat respectable grade of B- in that category. We would argue that one of the most critical factors in assessing an inspection report is deficiencies. In this regard, we especially look at the severity of the deficiencies. This particular nursing home received 7 deficiencies on its inspection report. We are also concerned by the fact that this facility was flagged by CMS for possible abuse or neglect. We prefer to avoid facilities with this designation. Please do your best to find an alternative nursing home.
Nurse Quality
We also ended up giving this facility a fairly respectable grade in the area of nursing, where it received a grade of B-. Our nursing grade consists of many datapoints, many of which are tied to quantities of nurse staffing. This facility provides only 0.4 hours of nursing care per patient daily. This is a somewhat uninspiring figure as it is well below the national average. Finally, despite not having the most impressive total nursing hours, this place performed admirably in the area of preventing major falls. This is typically a reliable indicator that a nursing home has sufficient quality controls in place.

Long-term Care Quality
The next highest category we awarded this facility in any category came in the area of long-term care, where we awarded it a grade of B-. Long-term care scores are based on the facility's quantity of nursing care. This includes a broad scope of personal care services, ranging from assistance with activities of daily living to routine medical services. After we finished assessing the amount of nursing care, we turned to the nursing home's vaccination statistics. We were a bit concerned this nursing home vaccinated only 33.510635 percent of its patients against pneumonia. On a more positive note, this nursing home was able to limit hospitalizations. Indeed, it had only 0.02 hospitalizations per 1,000 long-term resident days, which is a remarkably low figure.
Short-term Care Quality
The final area we assessed was short-term care, which turned out to be a weak spot for this nursing home. We gave this facility a grade of D for the area of short-term care. In the area of short-term care, we strive to qualify measures of a nursing home's rehabilitation services. We assess the facility's skilled nursing services, including those performed by registered nurses and physical and occupational therapists. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The last datapoint we looked at in this area is the percentage of residents that ultimately returned home from the facility. This nursing home struggled quite a bit in this metric as well, with just 3.2 percent of its residents returning home. Unfortunately, this was well below average.
