Facility Inspections

Grade: A-minus

While this nursing home's overall grade was as bad as it gets, it actually really excelled in the category of inspections. In fact, we awarded it an A- for that category, which is one of our highest scores. We take several aspects of a facility's inspection report into consideration in computing our inspection ratings. One of those factors is deficiencies. It should be noted that the severity of deficiencies is arguably more meaningful than the number of deficiencies, as some of these end up being relatively insignificant. While this facility had some minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are those found in categories G through L. This tells you that the inspectors did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A few minor deficiencies are not necessarily the end of the world.

Long-term Care Quality

Grade: D

Unfortunately, this facility's second highest score was long-term care. However, with a grade of D in this category, it still performed relatively badly. If you are seeking anything other than rehabilitation, you should take a close look at long-term care grades. Once we assessed the amount of care provided by nurses, we next considered the facility's vaccination records. We were pleased to find that this nursing home vaccinates 99.00745 percent of its residents against pneumonia. Surprisingly, this nursing home was able to limit hospitalizations. With only 0.01 hospitalizations per one thousand long-term resident days, this facility has fewer hospitalizations than many nursing homes. Unfortunately, some of its other scores in this category weren't as favorable as these.

Short-term Care Quality

Grade: F

Unfortunately, this facility received an F for its short-term care grade. Our short-term care grades are arguably more important for patients in need of rehabilitation services. Rehabilitation typically utilizes higher levels of highly-skilled nursing services. This includes not only nursing services, but also physical and respiratory therapy, as well as other variations of therapy. With its score in this category, we were not surprised to learn that this facility is well below average in terms of the number of physical therapy and registered nurse hours offered to its patients based on the measures we assessed. Lastly, we assessed the percentage of patients that returned home from this nursing home. This facility didn't fare well here either. In fact, we found that just 0 percent of this facility's patients were able to return home. This figure was quite a bit off the national average.

Nurse Quality

Grade: F

The next area we assessed is nursing, in which this facility was given a rock bottom score in this area as well. We gave it an F in this area, which is obviously somewhat alarming. Our nursing score looks at the nursing home's level of nurse staffing. We factor in both the levels of training of the nurses as well as the amount of time spent with patients. With a meager 0.2 hours of nursing care per patient each day, this place's nurse staffing levels were well below average. Finally, we also assessed some nursing quality measures in determining our nursing scores. This nursing home performed well when it comes to avoiding major falls and pressure ulcers.
